At approximately 11:54 a.m. LPA Laureano inspected the outdoor preschool yard and observed S1 and S2 supervising 20 children. Based on LPA's observation 2 staff members providing care and supervision to 20 children in the outdoor yard and staff file review, the allegation of staff operating beyond the terms and conditions of the license was substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. The deficiency was cited on the Complaint Investigation Report (con't) LIC 9099D.
LPA Judy Laureano informed facility representative Maureen Morrison that this report dated 12/10/2024 document 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Laureano informed the facility representative Maureen Morrison to provide a copy of this licensing report dated 12/10/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Upon on receipt of this report, the director shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
An exit interview was conducted and a copy of this report along with the Appeal Rights were explained and provided to Maureen Morrison, Center Director. |
