On 05/27/2026 Licensing Program Analyst (LPA) Lisa Clayton arrived announced at the Loyola Marymount University Children’s Center to conduct a Case Management – Licensee Initiated inspection regarding the room change of children in care, to the college student housing Resident Hall – Leavy 6 building due to planned construction to the Child Care Center which began May 2026, with an expected end date of August 2026. LPA met with Associate Director Grizel Lopez who provided a tour of the facility. There were no children in care

The Licensee is requesting a capacity of 24 infants, ages 0 – 2 years. Daycare will be conducted Monday – Friday/ 8:00 a.m.– 6:00 p.m. There is a fire clearance on file, approved on 03/09/2026, conducted by Gabriel Lopez, of the LA City Fire Department, Institutions Unit.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger.

Carbon monoxide and smoke detectors were observed in operable condition.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometer.

Age-appropriate furniture and equipment was observed in good repair. LPA observed toys for infants and toddlers that provided auditory, visual, tactile stimulation and manipulative skills

Cots were observed for napping. Director was reminded that bedding must be removed and stored separately when cots are stacked. Cots were observed in good condition.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Cubbies were observed for children’s belongings in each classroom.

Drinking water will be provided through filtered water faucets. Facility has completed lead testing for the water used for food prepping and drinking.

Windows were in good repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

LPA observed trash cans used for solid waste with tight-fitting lids.

Disinfectants, cleaning solutions, toxins or other poisons were observed inaccessible to children in care.

The Admin. office will be used for isolation of ill children. Associate Director agreed that there will be a cot, mat, or crib readily available for ill children and that these children must be under direct visual supervision at all times.

The facility was equipped with a working telephone. The classrooms have fans, windows and central heating.

The classrooms were clean in good repair.

Parents and authorized adults will sign children in and out with a QR on the Line Leader application. Associate Director was reminded that sign in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the department or its representative.

FOOD SERVICE:

Aramark (Loyola Marymount food service provider) prepares and provides and all meals. The center shall provide morning and afternoon snacks and lunch. Associate Director was reminded that there must be an adequate supply of foods readily available in cases of emergencies.

In each unit, LPA observed a kitchen with a sink, storage for food and a refrigerator with a thermometer. Toxins or chemicals were stored separately, and LPA did not observe foods during today’s inspection. LPA Clayton observed child safety gates in each kitchen, making the stove inaccessible to children in care.

RESTROOMS

Each classroom has 2 sinks, 1 potty chair and 1 toilet available for children's use only. The Bathtub in each classroom has a child safety gate installed, making the bathtub inaccessible to children in care.

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water. Changing tables were within reach of a sink, padding surfaces were at least one inch thick, covered with washable vinyl or plastic, with raised sides at least three inches high.

Applicant was informed there shall be a written toilet-training plan for infants being toilet trained per Title 22, section 101428 - Infant Care Personal Services

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in good condition.

The outdoor activity space was enclosed with a 4 foot or higher gate/wall. LPA Clayton observed a gate outside of classroom 313, making the emergency exit inaccessible to children, a gate in the outdoor area making the elevator and maintenance closet area inaccessible to children, and a gate at the walk-up entrance to the childcare area making the childcare entrance inaccessible to college students. LPA Clayton also observed additional security cameras in the outdoor area.

Grass was observed for cushioning. Water will be made readily available outdoors using filtered water pitchers and children use their personal water bottles for drinking.

LPA observed adequate shading and benches for resting.

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water should be removed immediately after each use.

No hazardous conditions were observed during today’s inspection.

Unit 308: Room A 154.76 sg. ft, Main Entry area 274.18 sq. ft.

Unit 310: Room A 167.87 sq. ft, Room B 157.35 sq. ft, Main Entry Area 237. 40 sq. ft.

The Indoor measurements contain a total of 991.56 square feet, which will accommodate Licensees request for 24 infants.

The maximum outdoor capacity equals 50 children at one time.

LPA Clayton advised Associate Director to request a waiver for the shared outdoor space PURSUANT TO AB 2131 CHAPTER 910, STATUTES OF 2022; 101804(e) The department may approve alternate use of outdoor play space to achieve separation from preschool children or infants if necessary to protect the safety of children in care.

The following closing statements were discussed with Associate Director Grizel:

Associate Director Grizel was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
