On 02/12/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Education Coordinator Elizabeth Mora and School Readiness Coordinator Joe Vega-Smith, who granted access to the facility. The purpose of the inspection was to conduct an unannounced case management inspection for a UIR received at Palmdale RO on 01/23/25. Upon arrival, there were 134 preschool age children in care, along with 11 Teachers.

Description of Incident: On 01/17/25, child #2 hit child #1 in the forehead with a toy which caused a laceration on child #1 forehead.

During the inspection, LPA interviewed staff involved and child #1.

Based on the information obtained, further follow up is needed to interview child #2. An unannounced case management inspection will be conducted at a later date.

An exit interview was conducted, and a copy of this report was provided to Elizabeth Mora and Joe Vega-Smith, along with a copy of Notice of Site Visit.
