LPA observed the menus from September 23 to 27, September 30 to October 4, October 7 to 11, October 14 to 18, November 4 to 8, November 11 to 15 and November 18 to 22. LPA observed AM and PM snack being served 2 components and lunch all 5 components. The menus indicate different vegetables and fruits being served. LPA observed reporting parting sprout about messages of what the child ate. LPA observed different meals served daily for lunch and include vegetables and fruits not just green beans. The facility is serving the minimum amounts of food components for AM/PM snack and lunch.

LPA observations, interviews conducted, and documentation reviewed did not support nor confirm the above allegations/allegation. This agency has investigated the complaint alleging staff are not offering the required food components to children in care. At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time.

Exit interview was conducted with director Maria Goldman. The director was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
