On 02/01/2023, Licensing Program Analysts (LPAs) Joselito L. Del Mundo and Carol Heath met with Director Aimee Ward to conduct an unannounced Annual/Random inspection. This Infant program is conducted on La Petite Academy school grounds. The facility has a total of 2 infant classrooms. During todays inspection. Infant rooms has 12 children (6 in each infant room). Present today were 12 children, 5 teachers. Teacher/Child Ratios were observed for each classroom. Teacher/Child ratios are met according to Title 5 of the California Code of Regulations and Title 22 Regulations. Children enter through the school gate that leads to the playground. Incidental Medical Services (IMS) were discussed.
· Physical Plant
LPA toured and inspected the infant classrooms. Furniture and equipment were inspected for age appropriateness and good repair. LPAs observed a sufficient quantity of cots (8 pieces) available for napping children.

LPAs observed cleaning solution are accessible to the children. Per Director Ward, she will keep the cleaning solutions under the diaper change area cabinet and secured with a safety lock.Trash cans have lids. Unused outlets are covered with plastic cover and are inaccessible to children in care. The infant classroom refrigerator has baby bottles that are properly labeled for infants. Infant’s personal belongings are place on top of the shelves. LPAs advised Director Ward to remove baby walkers in the facility.

The telephone service was verified. Heating, lighting, and ventilation are adequate. LPA observed age-appropriate toys and materials. There are fire alarms and carbon monoxide detectors located by

the doors of each classroom. Fire extinguishers are maintained properly.

· The outdoor play equipment was inspected for health, safety, cushioning material, good repair, and age appropriateness. The play area has rubberized cushion and concrete. Per director, children conducts missing and intruder drills and were accompanied by the Safety Captain. There is a designated Safety Captain in the Center. The Safety Captain inspects buses, playgrounds and backpack. The play area was inspected and found outdoor play equipment. During Summer, children play outdoor in the morning and in the afternoon for forty five minutes.

LPAs observed a first aid kit. The tweezer is missing in the first aid kit. There is a Fire Drill/Earthquake log posted in the office and a current Roster available.

· The parent board was reviewed and has all the required forms posted. Fire/earthquake drills are current. Per Director Ward, they do not administer incidental medial services. Notification of Parents Rights needs to be updated with regard to the Departments address.

Sign-in and out sheets were reviewed. The facility uses paper sign-in sheet. The Teacher to child ratio was observed and staff names were recorded. LPAs observed a parent board consisting of licensing-required postings. Fire/earthquake drills current.

· Staff Records/ Children Records
Children's records and staff records were reviewed. LPAs verified there is at least 1 staff person present with current CPR and First Aid Training (exp. 10/2023). All staff complete the Mandated Reporter training every 2 years except for one teacher. All staff receives Measles, Pertussis, and Influenza vaccination. All staff has required fingerprinting associated with the facility.
LPAs reviewed 10 children's records, LPAs observed 10 children's files contained all required licensing documents.

· Staffing Ratio and Capacity
Upon arrival, LPA observed 2 infant classrooms in use with a total of 12 infants and 6 in each classroom, with 2 teachers in each classroom.
· Food Service
There is a clean, fully equipped kitchen (off-limits) with a refrigerator, freezer (1), and microwave oven. Allergy lists are posted in the classroom. The facility provides breakfast, lunch and afternoon snacks which are carefully determined with regard to serving size per children’s age. Menus are posted in the office hallway. LPAs observed an appropriate amount of food and snacks. Per Director, cleaning solutions and other chemicals will be kept in a cabinet above the sink and secured with a safety lock. Soaps, detergents, cleaning compounds, or similar substances are stored separately from food supplies.

Drinking water is available inside the classrooms in the form drinking fountain. LPAs observed a sufficient quantity of cots available for napping children.


The following general information was discussed during this inspection:

The incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, childcare providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided according to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed

the initial mandated reporter training. Staff was informed that they need to take mandated reporter training.

Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184


**Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). In addition; all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of the report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt. Staff is aware of the required forms for children's files and forms that shall be posted after licensure.

**Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children.

**Parent Board contained all documents that are required to be posted according to Title 22 Regulations. Staff is certified in Pediatric CPR and First Aid (exp 10/2023). Sign-in and out sheets were inspected and contained full legal signatures. There is a fully stocked first aid kit available and a fully charged fire extinguisher and carbon monoxide detectors in place. The fire Drill/Earthquake log is posted.

Deficiencies cited: (See LIC 809D). The following Type B deficiencies are being cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety codes.
