On 11/12/2024 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced complaint investigation for the delivery of findings regarding the above-mentioned allegations. LPA Laureano was greeted by Isamar Galvan, designated director, and stated Director, Pamela Crissman, is currently on her way in.

LPA toured the facility inside and outside and observed 45 children 6 staff members providing care and supervision. LPA observed and inspected all 5 classrooms on the first floor and the outdoor space. LPA observed the kitchen area, the pantry and observed water dispensers and bottled water stored in the space. Per I. Galvan, filter water is delivered to the facility every two weeks.

LPA toured and inspected the second floor that is NOT used by the licensed program; storage rooms and two classrooms.

On 08/28/2024 Licensing Program Analysts (LPA) Judy Laureano and Cristina Castellanos arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations.

Upon arrival, LPAs met with Director Pamela Crisman and discussed the purpose of the visit. LPAs toured the facility both indoors and outdoors and observed no children in care. Director confirmed that facility is currently not open to children. Facility will reopen on September 3,2024. Director stated facility was preparing the classrooms for the beginning of the school year. Present during today’s inspection was the director and 8 staff.

On 9/5/2024 Licensing Program Analyst (LPA) Judy Laureano arrived at the above mentioned facility for the purpose of investigating the above mentioned allegations. Upon arrival, LPA met with Director Pamela Crisman and Isamar Galvan and discussed the purpose of the visit. LPA toured the facility both indoors and outdoors and observed 43 children and 6 staff members providing care and supervision. LPA requested and reviewed children’s file, staff file, children’s roster, and staff roster. LPA initiated staff interview and children’s interview.

On 10/30/2024 Licensing Program Analysts (LPA)s Judy Laureano and Brittany Lovest arrived at above mentioned facility for a subsequent visit. LPAs were greeted by Isamar Galvan and Pamela Crisman and discussed the purpose of the visit. LPAs toured the facility both indoors and outdoors and observed 39 children and 5 staff members providing care and supervision.

On 11/12/2024 all interviews with relevant parties were concluded.

Based on LPA Laureano’s multiple observation, interviews with staff, parents and children and document review, no information was disclosed that facility is in disrepair and/or that toys are not cleaned and sanitized. There is not enough information to prove or disprove the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the above allegations did or did not occur. Therefore, the allegations are found to be unsubstantiated.

An exit interview was conducted Isamar Galvan, designated director and copy of the report and Notice of Site Visit was provided.
