On 05/15/2024 at 8:40 AM, Licensing Program Analyst (LPA)Katrina Chicote conducted an Unannounced Required – 3 Year inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Oksana Danylchenko, who allowed entry to facility and guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present were two Assistants who are cleared. There was eight day care children, which includes three infants, present during today’s inspection. Additional two children arrived during inspection, one of which was infant, total census of seven daycare children and three infants for today's inspection. Licensee states that there are currently 11 children enrolled, LPA reviewed Facility Roster with 11 children listed. Per Licensee, the facility’s hours of operation are 7:00 AM to 6:00 PM, Monday thru Friday. LIC 126 was provided to Licensee. Required postings were observed by facility entrance. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 01/22/2024.

This is a single story home which consists of two bedrooms and one bathroom, kitchen, Day Care Space, and Dining Room. There is a fireplace in the home that has been completely blocked off by a diaper changing table making it off limits at time of inspection. Areas used by the children include the Day Care Space (located upon entry of the home), Dining Room (located to the right of Day Care Space), Bathroom 1 (located down the hallway), and Bedroom 1 for Infant Care Space (located next to Bathroom 1). Per Licensee, areas off limits to children and parents include Kitchen and Bedroom 2. LPA observed that there are physical barriers separating areas used by children and off-limits area at time of inspection.

Per Licensee, the children will have access to backyard for outdoor play. LPA observed backyard is accessible through door in Infant Care Room. At 8:45 AM, LPA observed backyard has soft flooring that is maintained and adequate perimeter fencing through-out the yard. LPA observed a variety of age appropriate toys free of loose and sharp parts available for the children in the backyard such as plastic dramatic play and climbing equipment, a shelf filled that is fixed that has balls and other play equipment. LPA observed all trees, shrubs, and plants are maintained at time of inspection.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPA observed there is an AC wall unit on wall by window in Day Care Space and wall heaters in the home that are barricaded making it off limits at time of inspection. There is a working telephone maintained in the home. At 8:47 AM, LPA observed Day Care Space to have a variety of age appropriate toys, free of loose and sharp parts such as soft plush toys, puzzles, plastic manipulatives, and books. LPA observed Day Care Space to have two sofas and toddler beds available. Per Licensee, this area is also used for sleeping. At 8:50 AM, LPA observed Dining Room to have age appropriate table and chairs. Per Licensee, she provides food to children in care and use Dining Room for eating. LPA observed children eating in Dining Room during inspection. At 8:52 AM, LPA observed Infant Care Room to have four pack-n-plays available for infant sleep. LPA observed fitted sheets on mattress, no bumper guards, pack-n-plays to be placed in room as to not block access to exits/entrances, and to not have anything hanging above them. LPA observed one infant inside a pack-n-play who was awake at time of inspection. LPA observed diaper changing table in Infant Care Room at time of inspection. Licensee states Infant Care Room will be used as an isolation area for sick children only when infants are not in the room. At 8:55 AM, LPA observed Bathroom to have operable equipment, adequate supplies, and general area safety and sanitation at time of inspection. LPA observed Bathroom's drawers, cabinets, and medicine cabinet and did not observe any hazards at time of inspection. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. LPA observed these items stored on high shelves and in off limits Kitchen area at time of inspection. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the size, age, and abilities of children in care.

At 8:55 AM, LPA observed the valve on the required 2A-10BC fire extinguisher indicates fully charged with proof of purchase receipt for 10/24/2024. LPA tested smoke and carbon monoxide detectors and observed to be in operable condition at time of inspection. LPA observed First Aid kit in Day Care Room and was inventoried for supplies. Licensee has current Pediatric First Aid and CPR valid until 02/2026. Licensee completed AB1207 Mandated Reporter training on 06/23/2021, which is not current at time of inspection. LPA discussed requirement for completion of this training for her and all Assistants. Required forms were discussed and provided. Proof of immunization against influenza, pertussis, and measles was not readily available during today’s inspection but was discussed. Children records and required licensing forms were reviewed for completeness.

LPA did not observe any pets at time of inspection and Licensee confirms she has no pets. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA observation did not see any bodies of water around the premises or firearms/weapons at time of inspection.

The following were discussed:
NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

MyChildCarePlan.org – Centers and Family Child Care Homes Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited only advisories given on 05/15/2024.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Licensee, Oksana Danylchenko.

During the exit interview, the Licensee Oksana Danylchenko, confirmed that there are no Registered Sex Offenders (RSO) living in the facility and LPA completed the RSO profile in FAS.
