25 | On 01/14/2025 at 3:33p.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection follow up On an UIR and LIC 624 Form and that was reported to the Regional Office. LPA met with La Shaye Davis, Regional Site Director and informed the nature of the visit. At the time of the visit there were a total of 8 children and 2 teachers.
According to the UIR, that was submitted On 12/24/2024 the Family Advocate, Leslie Avila, was making a routine phone call to address final documents needed for the child to officially start school on January 7, 2025. During the phone call mother wanted to make the school awre that when child started school she will display aggressive play with toys and may urinate on herself because she has witness Domestic Violence on several occasions inclusive of arguments between the parents and two incidents of father choking mother. One in 2023 and once in 2024.
LPA Whitmore was able to speak to the Family Development Advocate regarding the UIR. Family Advocate stated that the child has not started school, The Family Advocate is waiting for medical authorization, Mom stated that child has allergies and the doctor sent a form. Family Advocate explained to LPA about the Second Phase of the Registration. A Family Assessment was conducted with mom and also resources have been provided. Family Advocate does not know at this time when child will be able to start. LPA Whitmore was unable to view the child's file at the facility because child has not started. Based on the information obtained there were no violations of Title 22 Regulations.
