On 07/18/2025 Licensing Program Analyst (LPA) Dawn Dowling conducted an unannounced Annual Required Inspection and met with Licensee, Toni Tyre. Days and hours of operation are Monday through Sunday, 24 Hour care.

LPA toured the inside of the home and a census was taken. An Entrance checklist (LIC 126) was provided to Licensee at the start of today's inspection. Licensee confirmed that the living room, den, bathroom 2 (1/2 bathroom located behind the kitchen), bedroom 3, dining room, kitchen are primary childcare areas. Off-limits areas are :Master bedroom and backyard (currently under construction due to sprinkler issues) Children use front yard (gated) and go on field trips for outside play.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition on the premises. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible. There is a defunct fireplace in the living room and is made inaccessible as it is sealed off.

The fire extinguisher was serviced 07/02/2025. Smoke Detector and Carbon Monoxide were tested and operable. First Aid kit was complete. There is adequate heating and ventilation for safety and comfort. There are no stairs in this home. Safe toys and play equipment are observed.

The outdoor play area in the front yard is fenced and there are no hazards to children present. Capacity as specified on the license is being maintained.

LPA Dowling observed 3 children in care during today's inspection. Licensee resides in home by herself, her daughter comes to assist when needed and is fingerprint cleared.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA Dowling observed following documents posted on bulletin board in living room accessible to parents:

Licensee has Disaster and Fire Drills-last one conducted on 06/13/2025.

Licensee’s Mandated Reporter Training was completed on 01/09/2025

Pediatric CPR/First Aid expires 06/2027

Assistant associated to the facility has current Mandated Reporter Training certificates on file as well as Pediatric CPR/First Aid, taken same date as licensee.

Licensee provides meals and snacks for children in care.

Children have cots to sleep on, they bring their own bedding, which is kept in their cubbies. If a child has an accident bedding is immediately removed and bedding is changed and washed.

Isolation area is the back bedroom if a child is sick in order to keep away from other children in care.

LPA Dowling observed children in care were in caring and nurturing environment and treated with dignity and respect.

Licensee provided declination of flu shot for herself and assistant. Licensee was unable to locate immunization record for herself and assistant, will email to LPA Dowling upon locating it. Licensee under impression that once Department received and viewed immunization record that Department did not need to observe again. LPA Dowling advised Licensee to make file like she has for the children in care for herself and assistant so that when licensee comes all documents would be readily available. Licensee will be issued a Type B citation for immunization.

State law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category. LPA did not observe any prohibit items in home.



Criminal Record Statement Family Child Care Homes
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA Dowling advised licensee infants must be monitored every 15 minutes when sleeping, their breathing, temperature and color of skin should also be monitored. The licensee is advised to never shake a baby to prevent Shaken Baby Syndrome.

Only children eating may be in high chairs and that car seats are utilized only for transportation.

Medication :
LPA Dowling discussed medication with Licensee, currently Licensee gives medication that has been prescribed by child's Doctor and it is kept separate with child's name and instructions for giving medication on the label. Medication is inaccessible to children in care. Medication was discussed
Incidental Medical Services (IMS) policy

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.


Type B deficiencies were cited during today's inspection (see LIC 809Ds). The Licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days.

Exit interview conducted with Licensee. A copy of this report, notice of site inspection, Appeal Rights (LIC 9058), were given and explained during this inspection.
