25 | On 11/20/2024 at 8:26a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 11/8/2024 and was reported to the Regional Office. LPA met with Latrice Hearnes, Site Supervisor and informed the nature of the visit. LPA observed 18 Children in care with proper teacher/child ratios observed. There was a total of 6 staff.
According to the Unusual Incident/ Injury Report, at 10:40a.m. during outdoor time (C1) managed to push the side gate open that lead to the parking lot. Lead Teacher Arrica Fowler, saw the child and ran after her. Child was caught outside of the parking lot gate and brought back to safety inside the classroom. There was a total of 4 children present. A day-to day- substitute teacher was with the other 3 children. Parent was notified of the incident.
During the investigation, Licensing Program Analyst (LPA) Doris Whitmore was able to interview S1 and S2. C1 was absent today and the interview could not be conducted. During the interviews S1 was able to demonstrate from the time she was on the playground with another class and where C1 was found. S2 was able to demonstrate to LPA Whitmore the time she was in the classroom with another child until the time she came out of the classroom and noticed the gate was open and found C1. LPA Whitmore took pictures of the gate and a video of how the incident occurred on 11/8/2024. LPA Whitmore obtained a copy of the Care and Supervision Policy.
LPA Whitmore reviewed the child’s file, based on the information obtained and interviews further investigation is needed. Copy of report and Notice of Site Visit was issued to Site Supervisor Latrice Hearnes..LPA Whitmore informed the Site Supervisor that the Notice of Site Visit must be posted for 30 days.
