On 03/16/23 Licensing Program Analyst (LPA) Andrew Alemoh conducted an unannounced annual inspection at the above facility. LPA met with Director Amy Curtis. LPA inspected and toured the school age facility in accordance to the facility sketch during the time of this visit there are no school age children in care. Director states that school age children are present during, summer breaks, and winter breaks. This facility is a combination center and also possesses a preschool and infant license.

The facility consists of a total of 3 preschool classrooms;1 school age classroom and 2 infant classroom. At the time of visit, LPA observed zero school age children in care and there were 9 staff present at the facility today. The operation hours are 6:00 a.m.- 6:30pm M-F. Per the Departments Licensing Information System (LIS) all staff members obtain a criminal background clearance. The following areas were inspected and observed:

There are no bodies of water on the premises. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children were found to be inaccessible to children they are stored in a cabinet which was observed to be locked. Per, the Director Amy Curtis, there are no firearms or weapons on the premise.

Furniture and equipment were inspected for age appropriateness and are in good repair. Rooms inspected and are clean and safe. Telephone service was verified and operational. Heating, lighting, and ventilation are adequate.

Playground equipment: climbing structures, slides and other large play equipment is found to be securely anchored with adequate and resilient. LPA observed sand underneath the play structures. There was plenty of sand underneath the play structures and around the perimeter to absorb a fall of a child.

The surface of the outdoor activity space is in safe condition and free of hazards. Drinking water is readily available on the play yard. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc.

There is a separate area for isolation and care of ill children in the director's room. There are cots available for each ill child.

**Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.
**Sign in and out sheets were reviewed. The parent board was reviewed and has all of the required forms posted. Fire/earthquake drills current. Children's records and staff records were reviewed. LPA verified at least one staff person present has current Pediatric CPR and First Aid training exp 08/2024. Mandated Reporter expires on 07/23 .

LPA observed a large moveable wall in the classroom. Per director the wall is usually pushed against the wall however if there are more children in the classroom we move the wall to the middle of the classroom to create two separate classrooms. Per director she states that she makes the moveable wall secure.

There is a clean, fully equipped kitchen (off-limits) with a refrigerator, freezer, stove oven, microwave and dishwasher. This facility provides snacks and meals. Menus are posted in each classroom and in the main entrance area in which parent pick up and drop off their children.

Allergy lists are posted in each classroom and in cabinets in which food is stored. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food. The facility has ensured and establish measures to keep the facility free of flies and other insects or rodents.

There is an operating telephone in the office. LPA observed fully charged and operational 2A10BC fire extinguisher located in each classroom. This facility has one or more functioning carbon monoxide detectors that meets statutory requirements. All storage containers for solid waste, including moveable bins, have tight fitted covers that are kept on, and are in good repair.

Medications:
Children are inspected for illnesses as they arrive. Medications are kept in a safe condition, and free of hazards. LPA observed medications that are stored in a box which was locked.

A review of medication policy indicates that prescription and non-prescription medication is administered only with parent's written permission. Staff members will administer the medications. The director along with staff members check the dosage, and expiration dates on the medications. Medication is properly labeled and stored in its original container.

There is a separate area for isolation and care of ill children in the director's room. There are cots available for each ill child.

LPA observed that there are cubbies for children's belongings inside the classroom. LPA observed age appropriate toys and materials. Uncontaminated drinking water is readily available to children both indoor and outdoor via water dispenser; this facility has fountains and water dispensers.

LPA inspected and observed 2 clean bathrooms. The bathrooms are separated for the boys and girls. The bathrooms were inspected and found to be in sanitary condition. Each bathroom has 1 toilet and 1 sink. There toilets and sinks are functioning properly and are age appropriate. Each rest-room has adequate toilet paper, hand washing soap, and paper towels available for the children to use. LPA observed hot water had been capped off in both children's bathrooms.

Outdoors: LPA observed 3 separate play yards. There is black iron fence barricading all 3 play yards. LPA observed the play equipment was inspected for health and safety.
