On 09/06/2023 at 9:12 AM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote and Licensing Program Manager (LPM) Denise Gibbs. Facility is currently licensed for a capacity of 24. Licensing staff announced purpose of visit and initially met with Assistant Principal, Laura Ramirez. Facility is located on Harmony Elementary School campus in RM-150. The program currently operates a Full Day Program Monday thru Friday from 8:00 AM to 2:24 PM. Licensing Staff singularly toured the facility both indoors and outdoors. Required licensing postings were discussed with Facility Representative. LPA discussed the need to post Waiver for use of shared Kinder yard with Facility Representative at time of inspection.

At the initial start of the site visit, there is a total census of 11 preschoolers and a total census of two staff (S1 and S2) present.

Beginning at 9:25 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. LPA observed mats stored in Teacher's Office individually covered each with children's names labeled. Per Facility Representative (FR), linens are provided by the facility and are washed by laundry service weekly. Availability of drinking water was reviewed. LPA observed operable drinking fountains available in the classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available in classroom that has one toilet and one sink that were operable with adequate hand washing and toileting supplies, and to have general area safety and sanitation at time of inspection.

Snack/lunch menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by Los Angeles Unified School District. Licensing Staff toured food preparation area for safety, cleanliness, and adequate supplies. Licensing Staff observed disinfectants and cleaning solutions stored in separate room away from any food or food preparation areas. Children are provided with breakfast and lunch which is delivered from cafeteria. LPA observed large trash cans used to discard food did not have lid at time of inspection and discussed observation with FR.

A first aid kit is kept in behind white board in classroom and LPA inventoried for necessary supplies. LPA observed fire extinguisher and smoke and carbon monoxide detectors were present at the facility. LPA observed disaster drill log available, last drill conducted on 08/18/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children were not observed in the classroom. Per FR, there is one child (C1) with medication that is stored in school's Nurse's Office. At 9:47 AM, LPA discussed medicine storage with Nurse. Isolation for sick children is in school's Nurse's Office.

Beginning at 9:40 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed benches under shaded area and play structure to the right of outdoor play area that has cushioning material to absorb fall that is well maintained. LPA observed bicycles and other toys available for children to use outside that is well maintained. Required shade and fencing were inspected. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to water fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 10:40 AM, staff and children’s records were reviewed. Children's records were not available to be reviewed at time of inspection and requirement to have forms readily available for Licensing Staff were discussed with Facility Representatives.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. LPA observed Pediatric First Aid/CPR expires on 04/2024 for S1. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND Action Level Exceedance (ALE) have been corrected.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA advised that changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.


The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories given.

A notice of site visit was given and must remain posted for 30 days.


Exit interview was conducted and report was reviewed with the Facility Representative.
