32 | Infants are not swaddled while in care. LPA observe LIC 9227 Individual Infant Sleeping Plan form in the infants file. LPA observed sleeping log in the non-walker classroom. Director agrees to provide continuous training to staff to physically check on sleeping infants every fifteen minutes and document any signs of distress such as, flushed skin color, increase in body temperature, restlessness and labored breathing. Documentation for infants up to 12 months includes sleeping position if it is other than on their back. Staff-infant ratio requirements are being met while infants are sleeping.
Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.
Capacity and limitations as specified on the license are being maintained. Infant teacher One teacher and assistant were present during today's inspection. The name (Toke Smith) of the childcare center director or Annel Colmenarez is designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. In the non-walker infant classroom (0-13 Months), LPA observed 7 infants with one teacher and one assistant. In the walker Infant classroom (13 -24 Months), there were no children present today.
Facility maintains a ratio of one teacher supervising no more than 4 infants in care. LPA reviewed a sample of 10 children’s files and observed files were complete with contact information for authorized representative and/or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed 2 staff files for staff present today. Files included health screening, immunization records for influenza, pertussis and measles, mandated reporter, but did not include current CPR and First Aid. Menus are provided by parents to staff daily in writing and verbally.
