On 02/18/2026, Licensing Program Analyst (LPA) Lisa Clayton arrived at A Kid’s Place, to conduct an unannounced Plan of Correction visit. LPA Clayton was greeted by teacher Rosie. LPA Clayton observed 7 children in care children being supervised and cared for appropriately by 2 fingerprint cleared assistants/teachers.

On 12/02/2025, the CCC received a Type A citation for: 101227(a)(1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner.

a Type A Citation for: 101227(a)(16) - (a) In childcare centers providing meals to children, the following shall apply: (16) Pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.

a Type B Citation for: 101227(a)(19) - (a) In childcare centers providing meals to children, the following shall apply: (19) All food shall be protected against contamination. Contaminated food shall be discarded immediately.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Director will ensure that all parents of currently enrolled children receive a copy of the inspection report dated 12/02/2025, where a Type A citation was issued and place a signed LIC 9224 Acknowledgement of receipt of LIcensing Report in the childs file no later than 3/04/2026.

Director will provide parents of children enrolled over the next 12 months copies of the Licensing Report dated 12/02/2025 containing the Type A violation and maintain a signed LIC 9224 the above-mentioned children’s files.

a Type B Citation for: 1596.871(c) (1) (A) Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, pursuant to subdivision (f) of this section or Section 1522.7, from the State Department of Social Services prior to employment, residence, or initial presence in the facility. A person specified in subdivision (b) who is not exempt from fingerprinting shall be fingerprinted. The licensee shall submit fingerprint images and related information to the Department of Justice and the Federal Bureau of Investigation, through the Department of Justice, or comply with paragraph (1) of subdivision (h), prior to the person’s employment, residence, or initial presence in the child day care facility. The department shall not require the person to disclose their criminal history information prior to receipt of live scan results.

The Plan of Correction for the citations listed above have been met as evidenced by:
· Per Guardian, Licensee and assistant teacher Rosie have met the Criminal Record requirements.
· LPA Clayton inspected the food storage areas (refrigerator), and all food is safe and of the quality and in the quantity necessary to meet the needs of the children.
· The food storage cabinets were organized, and pesticides and other similar toxic substances were removed and stored separately, away from food, away from where food is prepared, and away from areas where kitchen equipment or utensils are stored.
· Food is protected against contamination and spoiled or contaminated food is being discarded immediately.

Deficiency Citations Cleared letters were created for the above citations.
On 12/02/2026, the facility received the following citations:

a Type B Citation for: 101227(a)(18) - (a) In childcare centers providing meals to children, the following shall apply: (18) All kitchen, food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.

The facility received a Type B Citation for: 101227(a)(6) - (a) In childcare centers providing meals to children, the following shall apply: (6) Menus shall be in writing and shall be posted at least one week in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

During today’s inspection, LPA Clayton observed that the menus were not posted, and although the staff have cleaned the kitchen, food preparation and storage areas, the plan of correction to watch the Food Service Requirements for Child Care Center video and provide a written declaration of understanding to the department no later than December 12, 2025 has not been met.

LPA Clayton reviewed the children’s files and observed signed Acknowledgement of Receipt of Licensing Reports - LIC 9224 forms for 3 of the 7 children enrolled.

Based on todays inspection, 1 (one) Type B violation is cited today (see LIC 809D) for the missing LIC 9224 forms.

An exit interview was conducted. A copy of this report, Deficiency Citations Cleared letters, the Type B citation and Appeal Rights were provided to Teacher Rosibel.

LPA Clayton posted the Notice of Site visit which is to remain posted for 30 days.
