25 | On June 4, 2024, @ 9:00 AM, Licensing Program Analyst (LPA) Miriam Cohen conducted a Case Management Incident inspection to follow up on the self reported incident that occurred on 05/20/2024 at Tutor Time Child Care Learning Center facility. The El Segundo Regional Office received the incident report on 05/22/2024. LPA Cohen met with preschool director, Lilian Medrano and stated the purpose for the visit. Upon arrival, LPA observed 60 children being supervised by 10 adults. All center staff members present during today’s inspection had fingerprint clearance and associated to the designated license number.
Based on the information that were gathered through interview, documentation submitted by the preschool director to the department, and physical plant inspection of the site of the incident, front lobby, it revealed that at the time of the incident, there were six children transitioning from Early Preschool classroom to Pre-K classroom #2. The classroom teacher (S1) failed to follow proper transitioning procedure and left one child (C1) in the Early Preschool classroom. Amanda Dimson, assistant director, was in the front lobby and heard the C1 turning the knob. Ms. Dimson went over to open the door and walked C1 to the Pre-K classroom #2. The student was left unattended for 30-45 seconds.
The director immediately discussed the incident with all staff members and reviewed counting protocols when transitioning from outdoors to indoors, and vice versa, with day-care children. A final written warning was issued for the teacher in charge (S1) during the incident. Director notified parent of C1 concerning the incident and provided information on the consequences for the teacher in charge of C1 during the incident.
As of 06/04/2024, C1 remained enroll in the preschool. Based on the available information: LIC 624 (UIR form), interview with the director, and physical observation of the site where the incident occurred, and consultation with management, it appears that the incident was the result of a Title 22 violation for lack of supervision. The content of this report was read and discussed in detail at the time of inspection with preschool director. An exit interview was conducted, and a copy of appeal rights was provided. |
