On 04/19/2024 at 12:00 PM an unannounced Required – 3 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and met with Facility Representative (FR), Margaret Delao, Co-Director. Facility is currently licensed for a capacity of 40, and per FR they currently have 22 preschoolers enrolled. LPA reviewed Facility Roster and is current at time of inspection. The program currently operates a Preschool Program with a Toddler Component Monday thru Friday from 7:30 AM to 6:00 PM. Per FR, they currently do not have any toddlers enrolled. LPA was taken on a guided tour of the facility of both indoors and outdoors. LPA provided LIC 126 and discussed required postings and placement of postings.

At the initial start of the site visit, LPA observed seven preschoolers with one teacher (S1) transitioning to nap time and ten preschoolers with one teacher (S2) in front Activity Room, total census of 17 preschoolers with three staff present at time of inspection. The facility is made up of four classrooms but at this time due to low enrollment are only utilizing two of four classrooms.

Beginning at 12:15 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed a variety of age appropriate toys used for dramatic play, math, and science well maintained free of loose and sharp parts. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected and observed laid out that it does not inhibit walkways or entrances/exits at time of inspection. Per FR, linens and blankets are provided parents and taken home weekly for washing. Availability of drinking water was reviewed. LPA observed water pitchers available for children and paper cups. Per FR, water pitchers are refilled from filtered water in KItchen. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available in front Activity Room with one toilet and one sink and additional bathrooms available in Preschool

Room with one toilet and one sink. LPA observed two diaper changing tables available with one within arms reach of a sink. LPA discussed regulation requirement and to use only the single diaper changing table that is within arms reach of a sink.

A first aid kit is kept in each classroom and LPA inventoried for supplies. LPA observed smoke detectors and fire extinguishers are present in the facility. Per FF, smoke detector is a dual carbon monoxide detector and LPA observed red light indicating it is operable at time of inspection. Disaster drill log was reviewed and last drill conducted was on 04/11/2024. Sick/isolation area in Office. Hazardous items including poisonous cleaning compounds were stored inaccessible to children, LPA observed these items stored in Kitchen area which is off limits through multiple doors.

Snack/lunch menus were reviewed and observed posted. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Per FR, facility provides AM Snack, Lunch, and PM Snack.

At 12:45 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed three outdoor play areas with metal structures well maintained and with cushioning material below to absorb fall that is well maintained. LPA observed surface of outdoor play area to be maintained in safe condition according to activities planned. LPA observed a variety of toys available including ride along toys and plastic playhouses. LPA discussed observation of maintenance of bicycles with FR at time of inspection. LPA observed Toddler Play Yard in the back that is kept separate from Preschool Play Yard. Per FR, at this time since there are no Toddlers enrolled Preschoolers are able to play in this yard. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play which LPA observed 2 of 3 to be operable at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA at time of inspection.

Beginning at 1:10 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults, all staff present were cleared and associated to facility at time of inspection. CPR card expires on 09/2025 for FR. Inspection of required forms made. 3 of 3 staff present did not have proof of the AB 1207 Mandated Reporter Training certificate on file. LPA discussed immunization requirements at time of inspection.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA referred FR to the Department website for lead: Lead Toxicity Prevention and Water Testing Information and provided resources during inspection.

The following were discussed:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation; this includes any changes, as soon as they occur, to physical plant involving construction, remodeling, telephone number changes and/or if you move from your location.



Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Margaret DeLao.
