On 11/21/24 Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Inspection at the above facility. At 1:30 PM LPA met with Reonna Pie, Assistant who stated Licensee Selina Lay was out. Also present in the home was Assistant Reonna Bryan. LPA provided LIC 126 Entrance checklist, and was guided on a tour of the home by Assistant Reonna P. LPA observed 11 children in care during inspection 9 of which were napping. Per Assistant hours of operation are 6AM-6PM Monday- Friday. Per assistant there are 14 children currently enrolled. Per Assistant there are no children in care requiring incidental medical services. Licensee provides breakfast, AM and PM Snack. Per assistant children bring lunch while home provides lunch on Friday’s only. LPA observed individual lunch pails for each child placed on table. Per assistant, overnight care is not provided. Transportation to and from school is provided. LPA observed all appropriate documents on licensing board (Facility License (LIC 203), Emergency Disaster Plan (LIC610a), last fire drill conducted 5/24, Notification of Parents' Rights Poster (PUB 394), Child Care Facility Roster (LIC9040). LPA also observed Licensee's current CPR (Adult/Infant/Child) and Adult/infant/child First Aid certifications with expiration date of 1/26. LPA observed licensee’s Mandated Reporter Certificate with expiration date of 1/26. LPA observed Assistant Reonna P's current CPR (Adult/Infant/Child) and Adult/infant/child First Aid certifications with an expiration date of 7/25 and current Mandated reporter certificate expiration date 11/25. LPA observed Assistant Reonna B's current current mandated reporter certificate with an expiration date of 9/26.

Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

This is a one-story home consisting of four bedrooms, three bathrooms, living room, kitchen, enclosed backyard and front yard and attached converted garage (Main daycare area). Per Licensee areas used by the children include the attached converted garage for activities, one bathroom in the garage, one bedroom connected to attached garage for sleeping and the enclosed front yard. Per Licensee, areas off limits to children and parents include: Three bedrooms (locked), two bathrooms kitchen, and enclosed backyard.

Per Licensee there are no poisons in the home. Detergents, cleaning compounds are kept in an off limit hallway, locked and inaccessible to children in care. Per licensee medication is stored in a locked off limit bedroom inaccessible to children in care. LPA observed a fully charged 3A40BC fire extinguisher with a purchase receipt of 11/21/24 on the wall of the main daycare room. At 2:25PM LPA observed a functioning combination smoke/carbon monoxide detector on the ceiling of the daycare bedroom. The home was observed to have central air and heating for safety and comfort. No wall heaters were observed. Per licensee there is an inoperable wood burning fire place in the offlimit living room. The home maintains telephone service via cell phone and landline. Per licensee there are no firearms on the premises. At 2:35PM LPA observed a jacuzzi in the offlimit enclosed backyard properly secured with a locked cover. At 2:40PM Licensee stood on top of Jacuzzi cover to show it properly supported adult weight. LPA observed body of water to be in compliance with Title 22 regulations.



LPA observed adequate toys and age appropriate learning materials in the main daycare room. LPA observed and mats as well as 4 play yards for sleeping. Per licensee all sheets and blankets are washed by Licensee weekly and as needed. At 2:10PM LPA observed infant in a moving swing placed on top of a table, presenting an immediate hazard to child in care. At 2:10PM Licensee removed child from swing, and removed swing from home. A type A violation will be issued for personal rights.

Kitchen area was observed to be clean and free of clutter and have no sharp objects or chemical compounds accessible to children in care. LPA observed kitchen knives to be stored in a high kitchen cabinet inaccessible to children in care. Per Licensee outside food is permitted in the home and brought labeled in children’s personal lunch pail. Bathroom children use is located in the attached converted garage and was observed to be clean and free of hazards. Per licensee potty training is provided.
Outdoor play area in the front yard is fully enclosed with no hazards present to children. LPA observed age appropriate toys and artificial turf for cushioning, proper shade as well as children's tables and chair.

Staff records were reviewed and observed to be complete. LPA reviewed 5 children’s files and observed 2 out of 5 to be missing immunizations. A Type B deficiency will be issued.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee Demetra Carter, confirmed there are no Registered Sex Offenders living in the home. LPA completed the RSO profile in FAS.

1 Type A deficiency and 1 Type B deficiency is being cited based on LPA observation in accordance with the California Code of Regulations, Title 22, see LIC 809D.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.
