On 01/14/2026, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced complaint inspection at the facility noted above, and met with facility representative. It was alleged that everyday staff wakes child 1 up from their nap.

LPA conducted interviews reviewed documents and the facility's file. These include staff members who were present during the nap period and individuals with direct knowledge of the incident. Seven interviews were conducted and three out of seven interviews consistently stated that a staff wake up all children from their nap by tapping them and using music. These three accounts were aligned in describing the circumstances and staff actions. During the interviews, staff also provided a physical demonstration of the actions taken; which include changing the music, turning on the lights, and tapping sleeping children. One out of the four remaining interviews did provide information that music is used to assist with waking up the children. The remaining interviews did not provide information that support or refute the allegation. Furthermore, documents obtained also supports the information gathered from interviews.

Per facility representative, they will review the regulation and complete a declaration stating that they understand the regulation and they will not wake the children. The declaration will be emailed to LPA J. Culbert by plan of correction (POC).

Based on interviews, three out of seven interviews consistently stated that a staff woke children up from their nap. This pose a potential health, safety, or personal rights risk to person in care.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of regulations, (title22) is being cited on the attached LIC 9099D. Appeal rights were provided and exit interview conducted with, facility representative. The Notice of Site Visit was given and must be posted for 30 days.
