32 | Based on the LPAs observations, interviews, and record review it was found that there is a teaching plan in place for substitute teachers as the facility currently has no permanent teacher or aide at the time of the occurrence. S4 has confirmed that there is a plan in place to ensure the children are not woken up and that all substitute staff are told at the start of the day. S1 and S2 both confirmed that upon assignment of the position they were notified that they are not to wake the children and that parents are to wake them at time of pick up if they are still asleep. Child interviews confirmed that they wake up on their own. Parent interviews confirmed that they have concerns about children being woken up but they are aware that they were given a plan of correction by licensing for waking up children and that staff is not consistent due to no permanent teacher or substitute teacher at this time.
S2 and S4 has confirmed that each child has a labeled mat and that there are 3 extra mats if a child has an accident. LPA observed each mat individually labeled for each child with bedding for each and 3 unlabeled mats for emergencies. Child interviews confirmed that they all sleep on a mat. Children were able to identify their mat and their sleep space. LPA observed this practice during observations. Parent interviews confirmed that they have concerns but allegations of sleeping on the floor are unsure. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
No deficiencies will be cited today 4/7/2026.
A notice of site visit was given and must remain posted for 30 days.
