13 | Licensing Program Analyst (LPA), V. Wheatley conducted an unannounced inspection regarding the allegation listed above at 1PM. LPA met with director Hiroko Kozuchi. The licensee Yoshio Aoyama was also on the premises however the LPA's communication was with Ms.Kozuchi. LPA observed 66 children napping in 6 rooms (three rooms on the south side of the building and three rooms on the north side of the facility). LPA observed the children supervised properly and within ratios.
LPA Wheatley and LPA Casillas conducted an inspection on 4/18/22 and interviewed staff based on a child's ear injury that was reported to the Department. The staff submitted declarations and denied the allegation and stated they did not observe the child's injury. LPA Wheatley and LPA Lillian Casillas interviewed other witnesses and was unable to obtain information to validate the allegation. LPA Casillas tried to obtain information from a viable witness however to no availability.
Based on the LPA's observations, information obtained and interviews which were conducted, there is not a preponderance of evidence to substantiate the allegation, therefore the allegation is Unsubstantiated. The unsubstantiated finding means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
