25 | On 4/22/2022, Licensing Program Analysts (LPAs), Veronica Wheatley and Lillian Casillas conducted an unannounced Plan of Correction inspection and met with the Director, Hiroko Kozuchi, at 2:20PM.
LPAs Wheatley and Casillas toured the entire preschool program and observed the children napping. The children were supervised properly and within ratios by staff. Director was supervising napping children.
At the end of the inspection, LPAs observed an adult female (Adult #1) who stated she is a piano teacher and teachers piano lessons for the preschool children. The Licensee's wife, Tomi Aoyama, was called from the other facility and arrived at 2:50pm. LPAs utilized Language Link to interpret in Japanese. Tomi Aoyama stated that the music teacher was her friend and she had an appointment with her today. Tomai Aoyama stated that she was thinking of using Adult #1 as a piano teacher in general. Tomi Aoyama denied Adult #1 was on the premises to teach the children today, although witnesses stated otherwise. The Adult #1 does not have a fingerprint clearance and is not qualified to supervise the children during piano lessons alone. According to the Director that Adult #1 usually teaches children with parents present. The director was informed that if the music teacher teaches the children alone she is required to have a fingerprint clearance. If the Department learns that the music teacher is teaching the children piano lessons without a fingerprint clearance in the future the licensee will be cited.
An exit interview was conducted with the Director, Hiroko Kozuchi. LPAs provided a copy of the report, as well as a Notice of Site Visit. |
