On 02/23/22 at 10:29am Licensing Program Analyst (LPA) Laticia Thompson conducted an unannounced visit to HART VILLAGE EARLY EDUCATION CENTER. LPA met with EDUARDO VILLEDA (Diretor). LPA advised the Director the reason for the visit today is to deliver the findings of the complaint received on 12/01/2021, regarding the allegations referenced above. LPA observed 10 children and 2 staff members In the butterfly room ages 4-5 years of age and 9 children with 2 staff members in the caterpillar room 2-3 years of age.

During the investigation, based on interviews with staff and witnesses of Allegation 1 revealed, there is sufficient evidence to support that lack of supervision resulted in daycare children engaging in inappropriate activity occurred. The preponderance of evidence standard has been met, therefore the allegation is found to be SUBSTANTIATED. California code of Regulations Title 22, Division chapter number are being cited on the Facility Evaluation Report (LIC 9099D).

An exit interview was conducted with the director, EDUARDO VILLEDA, in which this report was read to him. A copy of this report, a Notice of Site Visit (LIC 9213) and Appeal rights were given to the director.

The Director was advised that the Notice of Site Visit and a copy of this report must be posted at the entrance of the facility for a period of 30 days.

Placing children into smaller groups. Staff will be in a area in the room that staff can visually observe all children. Trainning and counseling the chidren on facial covering and appropriate behavior habits.

Based on LPA's interviews with a witness, staff member did not prevent nor observe C#4 from removing C#1 facially covering and kiss C#1 in the mouth. Which posses an potential Health and Safety or Personal Rights risk to children under care.
