On November 14, 2024, at 9:00 AM, Licensing Program Analysts (LPAs) Portia Bowden and Tiffanie Tran, conducted an unannounced Annual Required Inspection at the above facility. LPAs met with licensee, Gwendolyn Crews, explained the reason for our visit , provided LIC 125 (Entrance Checklist) and were guided on a tour. Per Licensee, the facility operating hours are Monday through Friday from 7:30 AM – 4:00 PM. LPAs observed all required documents on the facility licensing board (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A).

This is an Infant program serving ages 6 weeks to 18 months in babyland class. At 9:10AM, LPAs observed 0 children in care. Per licensee there are no children enrolled. This facility also serves Preschool children under facility #197413349 with 9 children.

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. At 9:15AM LPAs observed no operable Carbon monoxide detector, per Licensee the previous one broke, she will purchase a new one today. Per licensee, Linens are washed by the facility as needed and once a week (Fridays) . LPAs observed 7 Infant cribs located in nap room. All cribs were in good working condition with no attachments. LPAs observed 2 Age appropriate sinks and 2 age appropriate potties in good repair in the baby land classroom. General sanitation was observed.
Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. Per record review, LPAs observed negative lead test results conduct on 7/2023.

LPAs observed Disinfectants and cleaning solutions inaccessible to children. Per licensee, no food service is provided for infants at this time. Per licensee, food provided by parents are stored and labeled in refrigerator. ______________Page 1 of 3________________

All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

3 separate outdoor play areas were observed for each program. Playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

LPAs Reviewed 2 staff files, and observed that 1 out of 2 staff had an expired Mandated Reporter Certificate. LPA reminded Licensee that the certificate should be updated every two years. LPA observed Licensee’s CPR and Pediatric First Aid with EMSA with an expiration date of 2/25.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
