On December 20, 2022, at 2:00p.m., Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced follow-up complaint inspection and met with Director Paige Solomon. LPA disclosed the purpose of the inspection and was granted entry into the facility by office staff.

There were 26 children and five staff present when the visit began.

During the course of this investigation, LPA observed the facility, conducted interviews and obtained pertinent documentation. LPA observed staff supervising children at all times on multiple visits to the facility. Interviews conducted did not disclosed information regarding lack of supervision.

Interviews did not disclose a personal rights violation. Staff and child play a came called Zapp, where somone is "it" and has to "zapp" or catch other other children. During the game staff and children do have contact with one another, taging various approrpiate parts of body. Children interviewed state that the game is fun. No children disclosed information regarding staff flicking them. ------------------Page 1

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove
the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted. A copy of this report and appeal rights were discussed and left with Director, Paige Solomon, whose signature on this form confirm receipt of these documents.

On December 20, 2022, at 1:00p.m., Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced follow-up complaint inspection and met with Director Paige Solomon. LPA disclosed the purpose of the inspection and was granted entry into the facility by office staff.

There were 26 children and five staff present when the visit began.

During the course of this investigation, LPA observed the facility, conducted interviews and obtained pertinent documentation. Interviews with staff and documentation on the Bright Wheel application confirm that July 30, 2020, Parent was not notified about a bump on the head tha toccuried during the school day. LPA discussed injury reporting with Director who stated that the currently use Bright Wheel to notify parents of any incidents or injuries at the facility and they have not had any recent concerns.

Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be Substantiated. California Code of Regulations, (101226.3 Observation of the Child), are being cited on the attached LIC. 9099D.

Per Director, they are currently using the Bright Wheel Application to inform parents of any incident or injury that occur at the school. Facility also utiilized written reports that parents have to sign. Director will ensure she gets a respond from parents confirming receipt of message.

Based on record review the licensee did not ensure parent was notified about an injury of their child. This poses a potential Health, Safety or Personal Rights risk to children in care.
