On January 26, 2023, at 9:30 a.m., Licensing Program Analysts (LPA) Denise Gibbs and Franchesca White conducted an unannounced inspection and met with Director Peyton Solomon. LPA disclosed the purpose of the inspection and was granted entry into the facility by facility staff.

There were 19 children and 7 staff present when the visit began.

During investigation, LPAs observed the facility, conducted interviews and obtained pertinent documentation. LPAs confirmed in Pre-k 4/5 there were three unsecured wood planks propped up in the beams of the ceiling. Staff removed wood planks and discarded them during inspection. The carpet in the main classroom had dirt all around the edges and throughout the carpet. Carpet also was observed to have debris scattered throughout. Carpet was removed during inspection and replaced. LPAs observed loose wires from the television in the nap room close to the ground accessible to children. Wires were temporarily blocked my unused cots making them inaccessible to children. ------------------Page 1

Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be Substantiated. California Code of Regulations, 101238(a) Buildings and Grounds, are being cited on the attached LIC. 9099D.

The Notice of Site Visit (LIC 9213) – must remain posted for 30. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director. A copy of this report and appeal rights were discussed and left with Director Peyton Solomon, whose signature on this form confirm receipt of these documents.
