25 | On 12/12/2023 at 1:00 PM, Licensing Program Analyst (LPA) Katrina Chicote arrived at the above facility for the purpose of conducting follow up on Plan of Corrections (POCs) from five Type B citations issued on 10/19/2023. Census was taken. LPA arrived to facility during children's nap time.
During inspection, LPA toured facility and observed outdoor play area. LPA did not observe surface of cushioning material under structure of play area maintained at time of inspection. LPA observed holes previously observed still unpatched, per Licensee (over the phone) he has been attempting to get holes patched up and has been in conversation with the Contractor but has been having difficulties getting the Contractor to work with them. Licensee states this will be taken care of after the New Year.
LPA reviewed six staff files present during today's inspection. Record review reveals that all staff files reviewed have current required AB1207 Mandated Reporter training completion certificates on file, all personnel records missing LIC 503 now have them complete, immunization records were obtained by staff, and staff have education verification on file. LPA reviewed required documents during inspection and reminded Facility Representative that files must remain maintained and if new staff is hired their files must also be maintained. Clearance letters were issued in regards to deficiencies.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/12/2023.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representatives.
