On January 09, 2026, at 9:30 AM, Licensing Program Analysts (LPAs), Elka Chavez and Tyler Reyes conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPAs met with Applicant Representative, Linda White. At 10:00 AM LPAs were joined by Aniya Terry, Office Manager. Applicant, Trinity Unlimited Outreach is requesting to be licensed for 22 infant children 0-24 months in classroom 4 (Tiny Tots A) and 4 toddler children 18 to 36 months in classroom 1 (Tiny Tots B). Hours of operation will be Monday through Friday from 6:30 AM to 6:00 PM.

Present during today’s inspection were (15) children with (4) staff. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Linda White that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.

LPAs continued to tour the facility and measured new indoor and outdoor activity space. The total infant indoor activity space measured 413.21 square footage, which is sufficient to accommodate the capacity of (11) Children. At 11:00 AM LPAs observed the infant crib area was in the process of having the roof repaired due to a leak caused by recent rain. LPAs observed a fireplace without a barrier. LPAs observed tiles next to the door that leads to the outdoor activity space lifting off the ground. LPAs observed the infant indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, 1 feeding chair, cubbies, 7 cribs, napping cots/mats for infants who can climb out of a crib and other activity supplies for the children. LPAs observed a restroom with 1 toilet, 1 sink and 2 potty chairs within the Tiny Tots A classroom.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPAs observed the highchair to have broad-based legs; plastic seats are in good repair and trays lock onto chairs. The highchair is made of washable, moisture-resistant material. LPAs observed the changing table to have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a mesh retractable baby gate. This barrier is at least four feet high and allows for supervision of napping children. LPAs did not observe the gate to be made of sound absorbing material. Sleep logs, Needs and Services Plans and Sleep Plans are kept in the main office.

At 11:30 AM LPAs toured and measured the Tiny Tots B classroom. This classroom is currently designated to preschool license number 197416403. The total toddler indoor activity space measured is 143.43 square footage, which is sufficient to accommodate the requested capacity of (4) Children. LPAs observed the toddler indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, cots and other activity supplies for the children. LPAs observed a restroom equipped with 1 toilet and 1 sink within the Tiny Tots B classroom. Drinking water is available in the classrooms via filtered water dispenser and disposable cups. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/13/2025.

There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the front office. The ill children will use the middle/orange restroom.

The facility currently has 2 fully fenced playground areas. Fencing is wrought iron and is at least four feet high. The total square footage for infant outdoor activity space is 520.00 square footage, for only 6 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. LPAs observed shade is provided via structure in the infant outdoor activity space. The total square footage for the toddler outdoor activity space is 481.05 square footage, which is sufficient for the number of children being requested. LPAs observed the rubber floor mats were splitting

from each other. LPAs did not observe shade to be provided for the toddler outdoor activity space. LPAs did not observe outdoor age-appropriate toys on the playgrounds. Drinking water is available via water jug and children water bottles. LPAs observed a retractable gate in the infant activity area which is accessible to children. LPAs observed the gate retractable gate has chain that is accessible to children. LPAs observed the motor to the gate is accessible to children.
Applicant Representative, Linda White, was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Lead testing was completed on 04/19/2023.

The following corrections are needed prior to the issuance of the license:

• The Applicant needs the separation from the sleeping area to be constructed of sound-absorbing material.
• The Applicant will need to submit a waiver for staggered use of the infant outdoor space to obtain their requested infant capacity.
• The Applicant needs to repair the leak near the infant sleep area.
• The Applicant needs to repair the tiles near the entrance to the outdoor activity space
• The Applicant needs to place a barrier in the fireplace.
• The Applicant needs to provide share for the toddler outdoor activity space
• The Applicant needs to make the chain for the retractable gate inaccessible to children in care.
• The Applicant needs to fix the mats in the toddler outdoor activity space.
• The Applicant needs age-appropriate toys for the infant and toddler outdoor activity space.

Applicant Representative, Linda White, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Licensee representative, Linda White was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code

LPAs discussed the safe sleep regulations with applicant representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPAs also informed applicant representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.”
Applicant representative, Linda White was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given to Applicant Representative Linda White and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
