An unannounced, in-person Case management incident inspection was conducted on this date by Licensing Program Analyst (LPA) Jeanine Lipsey. The incident was reported to Community Care Licensing on 06/17/24 via phone. The incident which occurred on 06/12/24 was that a staff member (Staff 1) was allegedly seen on video pulling the hair of a child (Child 1).
Upon arrival, LPA was guided on a tour of the facility by Staff Mariela Cortez, to whom the purpose of the inspection was announced.
Census: There were twenty children being supervised by 3 staff.
During the inspection, LPA obtained a copy of the facility roster, Employee roster, Termination report, recorded video on LPA's phone, and conducted interviews with 2 staff and 1 child, and made observations of care and supervision of children.
Two of the staff interviewed described what they saw on the video: C1 was by a table, facing the table, playing with a tablet; then S1 walks up from behind and pulls C1’s hair and walks off; C1 then fans with their arm toward S1. When interviewed, S1 stated that the hair pulling was done playfully. No disclosures were made by any of the children interviewed. LPA Lipsey was unable to view the video of S1 allegedly pulling C1's hair because per S2, the video is unable to be retrieved due to the system being old and it erases after 3 days.
The termination report stated S1 was told not to return to Kinder Kid School as of June 13, 2024.
Exit interview conducted and report was reviewed with Director Michelle Moore. A Notice of Site Visit was given and must remain posted for 30 days.
