An unannounced inspection was conducted by Licensing Program Analyst (LPA) Roxana Lopez on 2/18/2026. The purpose of this inspection is to provide the findings of the complaint investigation which was received on 12/28/2025. LPA met with Facility Representative Anacani Gallegos to whom the purpose of the inspection was announced. Census was taken.

Throughout the course of the investigation, interviews were conducted with staff and parents. LPA also reviewed and obtained copies of children’s rosters and other documentation.

Per initial complaint report, the complainant reported that staff is not adequately trained. when staff are placed in the classroom to cover for other staff. During interview, complainant, stated that the staff who are covering classrooms are not given any information on behaviors, behavior plans or how to address any behaviors. ------------------------------------------------ pg. 1 of 2 -------------------------------------------------------------

In regards to allegation staff are not adequately trained. Staff # 1 disclosed that when there is any staff coming in to cover staff that are out- they are provided with the classroom # they are going into, a roster of the students, lesson plans and allergies are posted in the classroom. Per staff # 1 any behavioral issues or behavioral plans are not provided- once staff goes into the classrooms it is the permanent staff who informs staff covering any issues. Additionally, Staff # 1 stated that training's provided on behavior and how to handle behavior as well as resources provided are for official staff, covering staff have access to these training if they are present during the training. Staff # 2-4 disclosed that they assume that when they have staff covering the classrooms all the information regarding the classroom, behaviors and behavioral plans are provided by administration. Additionally staff # 2-4 disclosed that official staff get training's regarding behavioral management. Staff # 3 disclosed that some of this training is not age appropriate for the ages they serve or needs that are in the classroom.

LPA Lopez conducted interviews with parents no disclosures were made regarding this allegation.

This agency has investigated the complaint alleging Staff are not adequately trained. Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. The following deficiency listed on the attached LIC 9099D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiency that is being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Representative Anacacia Gallegos.

Per Head Teacher they will provide the information to the Principal. A written plan on how covering staff would be trainied will be submitted to LPA by POC due date of 3/6/26.

Based on interviews, the licensee did not comply in the section above in that S #1 disclosed that trainings are not provided to covering staff and S# 2-4 disclosed that they assumed information is given by admin staff. This poses a potential health and safety risk to children in care.

Per initial complaint report, the complainant reported that staff did not address the behavior of a child resulting on another child getting hurt. Per Complainant- staff does not address the behavior appropriately and a child was hurt. Complainant did not provide information on child that was hurt.

In regard to allegation Staff did not address child's behavior resulting in another child being physically harmed. Staff # 1- 4 corroborated that they have children in the classroom that need extra support and some behavior is age appropriate. Per staff # 1-4 child # 1 needs extra support and trainings and resources have been offered to parents. Staff # 2-4 disclosed that a teacher is always closed to child # 1 as they support child # 1 with communication skills. Per staff # 1-4 all children throughout the day are reminded of the rules, including keeping hands to themselves, books are read and activities are provided to better support children. Staff 1-4 corroborated that there has not been an incident where a child was hurt and needed a medical attention.

LPA conducted interviews with parents and children. Parent’s statements corroborate that they do not have any concerns with the facility or supervision and that they are provided with different training's and resources to better support their children at home. Overall, parents are happy with the care that their children receive.

During children interviews: Child # 2-4 disclosed their hair has being pulled by child # 1 however a teacher was present to help them. Children 2-5 statements corroborate that they like their school and their friends.

This agency has investigated the complaint alleging Staff did not address child's behavior resulting in another child being physically harmed.

Based upon the evidence as presented above, the allegation has been determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated.
