On 01/09/2024 at 11:30 AM an unannounced Required – 3 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with S4 who provided initial tour of the facility. Director arrived shortly after and continued rest of tour. Facility is currently licensed for a capacity of 12 infants. There is an Preschool Program license at the same facility (#197416636). LPA observed Preschool Program has separate entrance from Infant Program. The program currently operates Monday thru Friday from 6:30 AM to 6:30 PM.

At the initial start of the site visit, there is a total census of ten infants and a total census of three teachers present. The facility is made up of two classrooms: Infant Room and Toddler Room (max 2). LPA observed six infants with two staff (S1, S2) in Infant Room and four infants with one staff in Toddler Room. Sign in/out procedures were discussed and LPA observed all infants present signed in at time of inspection.

Furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean and age appropriate. LPA observed each classroom to have age appropriate equipment which includes low tables and chairs with rounded corners. LPA observed a variety of age appropriate toys made for tactile and visual stimulation as well as tactile skills. LPA observed high chairs available for infant feeding outside of classroom with broad based legs and made of wipeable material. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for infant's belongings. LPA observed wooden cribs against walls of Infant Room, placed so it does not hinder entrances/exits. Per Director, family's provide the sheets and facility does washing of bedding. LPA observed two infants sleeping in Infant Room in cribs with fitted sheet and no loose articles inside or hanging above. LPA observed Infant Room sleeping area to be in same location as Infant Activity Space, LPA discussed observation with Director at time of inspection. Availability of drinking water was

reviewed, LPA observed bottles stored in refrigerator in classrooms with individual infant names labeled clearly at time of inspection. LPA observed changing tables in Infant Room and one in Bathroom available through Toddler Room that meets Title 22 Regulation requirements. LPA discussed changing and sanitation procedures with Staff at time of inspection.

A first aid kit is kept in the Kitchen. LPA tested carbon monoxide detector which were observed to be operable at time of inspection. LPA observed smoke detectors and fire extinguishers are present at the facility. LPA observed fire extinguishers have been serviced on 10/2023. Disaster drill log was available, LPA observed last drill conducted on 11/15/2023. Isolation area for sick children in Director's office which LPA observed to have lounger. Hazardous items including poisonous cleaning compounds were stored inaccessible to children, LPA observed these items stored up high.

LPA observed snack menus posted. Per S1, facility only provides AM and PM snack and infants bring in lunches from home. LPA observed infants lunch boxes stored in shelves with individual names of infants and additional infant lunches stored in refrigerator in Kitchen with names labeled. Snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment.

Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed Infant Playground and Toddler Playground available in outdoor play area. LPA observed age appropriate toys free of loose and sharp parts such as push toys and surface of outdoor play areas to be maintained in safe condition for activities planned. LPA observed an Infant swing set with four swings to be placed on top of grass, serving as cushioning material. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles for drinking water outside. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Personal Rights of children were observed by LPA.

Beginning at 12:30 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 05/2025 for S1. LPA did not observe immunization records available for 3 of 3 staff present, LPA discussed requirement with Director. 1 of 3 staff present had current proof of completion for required AB1207 Mandated Reporter training certificate on file, LPA discussed requirement.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories.

A notice of site visit was given and must remain posted for 30 days.
