On 8/3/2022 8:45 am Licensing Program Analyst (LPA) Denise Miranda conducted a Case Management Inspection for a Plan of Correction (POC) that was cited on 7/28/2022. LPA met with Karen Avital, Director and discussed the purpose of this inspection. Around 9:05am LPA observed a Teacher#4 and Director with 9 children. Around 9:27am LPA observed 16 children with 2 staff and Director.

The following were cleared on this Inspection:
Criminal Record Clearance : One adult was present on 6/29/2022 without fingerprint and one adult was present without transfer and associated to the facility. On 6/29/2022 an incident happened and Director was not present.. Cited on 7/28/2022. On 8/3/2022 LPA observed that the two individuals were not present at the facility. Per Director, they will return only when they get clear status and been associated to Barefoot Preschool.
Teacher Aide Qualifications and Duties: On 7/28/2022, LPA observed that Director was not present and Staff#3 Teacher Assistant) was present with 4 children in care. Cited on 7/28/2022.
On 8/3/2022 Upon arrival, LPA observed that Director was present present with one Staff#3 (Teacher assistant) with 10 children in care.
Children files were reviewed for the LIC9224 Acknowledgement of Receipt of Licensing Reports for 7/28/2022. Director will submit no later than tomorrow (LIC9224) for 2 children that still missing, LPA agreed to receiving this documents no later than tomorrow 8/4/2022.
A Copy of Letter of Deficiency Citation Cleared was provided to Director.
At the time of the plan of correction visit the facility was found to be in substantial compliance. An exit interview was conducted, and a copy of this report along with notice of visit was given to Director Karen Avital.
