On 08/12/2024, Licensing Program Analyst Adrian Risher conducted a case management visit for increased monitoring. LPA met with Candice Lee, Administrative Assistant. LPA explained the purpose of the visit. LPA observed 6 children in care with 2 staff and 1 Director.

An earthquake occurred around 12:20pm while LPA Risher was conducting the inspection. The children were playing outside on the playground located on the side of the building. Staff and Administrative Assistant gathered the children and walked them to the playground in the back of the building. Staff were observed providing proper Care & Supervision to the children during and after the Earthquake.

LPA is following up on the Stipulation, Waiver and Order that was received on 09/08/2023. The facility was issued a probationary license from 09/08/2023 to 09/08/2026. LPA observed the Stipulation, Waiver and Order posted on the Parent Board and Probationary License.

LPA observed staff providing supervision to the children in care. The facility is operating within the required Staff to Child Ratio. LPA observed 6 children in care with 1 staff in Room 1. LPA observed 2 children with 1 staff in Room 7. Administrative Assistant stated the Director will cover the opening schedule during staffing emergencies. The facility is working on creating a Substitute Teacher list that can be referred to during staffing emergencies. LPA Risher recommended utilizing temp agencies for staffing as well. Administrative Assistant stated the facility plans to reach out to LA Southwest College's Child Development Center to collaborate on hiring emergency staff. The operating hours changed to 7am to 6pm mid-week during the week of 8/5/2024 to 8/12/2024. Before the change in the operating hours was implemented, the administrators discussed the new hours with the staff to confirm that the staff could work with the changes.

Administrative Assistant stated she works as the liaison to provide support in the transition of responsibilities and duties from the Administrators (Mr. & Mrs Lee) to the new Director. Administrative Assistant will provide copies of her International transcript to confirm if the classes previously taken meets the requirement for a

Full-time Teacher. Administrative Assistant is willing to be used as Emergency Staff if she meets the qualifications. Administrative Assistant is primarily present to help with the transition of responsibilities to the new Director.

The cameras installed in the facility are operating and the facility is being monitored at all times. The cameras are located in the following areas: entrance parking lot, the front door, every classroom, kitchen, library, nap room, side playground and the back playground.

LPA reviewed the following documents: sing in & out sheets, Facility Roster and Personnel Report. Each child present was properly signed in.

The following documents were requested from the facility: LIC 200 A (hard copy), Staff time-sheets, and Updated Roster. LPA received the LIC 200A, a sample of the Staff time-sheet for the week of 8/5/2024 to 8/12/2024 and the Facility Roster.

LPA Risher followed-up on the status of the required training which were included in the Stipulation, Waiver and Order. Administrative Assistant stated the facility owners (Mr. & Mrs Lee) have signed up for the training but the training has not been completed yet.

Facility will continue to be under increased monitoring on a quarterly basis during the probationary period.

There have been other contacts regarding pending documents with LPA Scott. The facility will follow-up with LPA Scott regarding those pending documents per the Stipulation, Waiver & Order .

Exit interview was completed with Candice Lee, Administrative Assistant. Appeal Rights will be provided.
