On February 5, 2025, at 2:30 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced Case Management Inspection – Plan of Correction at the above facility. LPA met with licensee, Naira Meliksetyan who guided LPA on a tour of the facility. LPA observed 7 children in care. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 1/29/2025 were corrected.

Licensing staff observed and reviewed the following:


· Licensee purchased 2A-10BC fire extinguisher

Letters of Deficiency Citation Cleared were provided for deficiency corrected.

A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and a copy of this report was provided to licensee, Naira Meliksetyan.
