Licensing Program Analyst (LPA), Jillinda Chandler, conducted an unannounced visit at the Perikhanian Family Child Care Home, on 02/10/2023, for the purpose of delivering findings for Complaint Control Number: 58-CC-20221115150216. Complaint investigations were conducted by LPA, Keyona Scott.
LPA Chandler met with Licensee, Anahit Perikhanian. LPA did not observe any children in care. Per Licensee, she does not currently have any children enrolled. Licensee informed LPA Scott that childcare services had not been provided for the last three to four years prior to November 2022, and licensee is in the process of looking for children to enroll.

All Adults present, residing, working and/or volunteering in the home have a criminal record clearance or exemption.

Based on interviews conducted and disclosures that were made, the allegation of "licensee did not properly supervise day care child" is UNSUBSTANTIATED, meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred.

An exit interview was conducted with the licensee. A copy of this report and appeal rights were discussed and left with the licensee, Anahit Perikhanian, whose signature on this form confirms receipt of these documents.
