Licensing Program Analyst (LPA) Alicia Bailey conducted an unannounced random annual (or Required) inspection. LPA met with Licensee Handra Davis at 2:40 pm who guided the LPA on a tour of the facility. At 2:50 pm during the inspection LPA Bailey discussed, assessed and noted individuals residing in the home. The facility Entrance Checklist for Family Child Care Homes was given to Licensee Davis. This is a one story home consists of 4 bedrooms and 1 bathrooms ,detach garage, front yard and backyard. Areas used by the children living room, one (1) restroom, family room, and Back yard. Per Licensee Davis and facility sketch, areas off limits to children include: 4 (four) bedrooms, and garage .The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. There no other license held at this facility. There are 2 children present during time of inspection. Licensee Davis states that there is currently 2 children enrolled in the facility . Facility hours of operation Monday thru Friday 6:00 am to 6:00pm. The facility ages 0 years old to 12 years old.

At 3:00pm LPA Bailey ask licensee Davis do you have a firearm, licensee Davis state no. LPA Bailey ask licensee Davis do you own a pet licensee Davis state no.

At 3:10 pm LPA Bailey observed a large television in the children room. LPA Bailey observed, age appropriate toys and napping equipment. At 3:17pm LPA Bailey tour the children bathroom. LPA Bailey did not observe any hazardous items in the bathroom.

At 3:30pm LPA Bailey toured the kitchen the counter tops were clean and free of clutter or standing food. Kitchen cabinets has safety lock and knives and medicine was in secure cabinet with safety lock. Licensee Davis provides breakfast, lunch and snack.

LPA Bailey observed there no swimming pool or body of water. At LPA Bailey observed fire extinguisher was last service in 08/20/2022. Licensee Davis state the last Earthquake & fire drill was conducted on 01/05/2022. First Aid supplies were inventoried. A review of medication policy, including administering, labeling, storage, and records was made.

The smoke detectors, and carbon monoxide was tested by licensee is in operable condition.

At LPA Bailey toured the front yard was inspected for hazard.

Licensee Davis current mandated reporter training certificate 08/22/2024. Licensee Davis PED F/A & CPR EXP. 08/22/2024.



At 3:38 PM LPA Bailey request the children files . LPA Bailey reviewed one children files, one file was compliance.

Licensee Davis was advised that regulation prohibits the smoking of tobacco in a private residence licensed as a family childcare home during the hours of operation. LPA Bailey reviewed with Licensee different item prohibited in FCCH, no baby bouncers, no infant walkers, No Johnny jumpers, no saucer chairs, no trampolines and any other item that falls into that category are not permitted in the facility.
Highchair are only use during feeding. Car seats are only use for transportation

Licensee Davis does not provide transportation. At 3:49 pm LPA Bailey also consulted with Licensee Davis regarding COVID-19 health and safety guidelines. At 3:56 PM LPA Bailey conducted staff interview with Licensee Davis.

At 3:58 pm, the following was discussed with the Licensee Davis

The following was discussed with Licensee Davis Individuals who are 18 years of age or older living in the home must be fingerprint cleared prior to licensure. Individuals within one month of their 18th birthday must be fingerprinted immediately. The existing, immediate $100 per individual Civil Penalty has been increased to an immediate $100 per day Civil Penalty, for a maximum of five days for the first violation and a maximum of 30 days for subsequent violations. If an individual has a clearance with the Department a criminal record clearance may be transferred. LIC 9182 Criminal Background Clearance Transfer Request may be used.

The licensee Davis is providing Incidental Medial Services currently.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Bailey reviewed Sudden Infant Death Syndrome (SIDS), Shaken Baby Syndrome, and safe sleep practices with licensee. *Infants should always sleep on their backs, mouths facing up.

Infant Needs and Services Plan: The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts was provided. LPA discussed PIN 20-24-CCP.

Overnight Care: There is no overnight care provided at the moment. LPA Bailey discussed the following: Licensee is aware that they must remain awake while children are awake. If children sleep in a separate area from the licensee, the door must remain open. If the licensee cannot hear children when they wake up, a video or audio device can be use.

LPA Bailey advise licensee Davis to review department website for current updates :

LPA Bailey advise that Licensee Davis review the Department’s training videos
at https://ccld.childcarevideos.org/ in regards to (personal rights/supervision ect.).

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
