On 11/14/2024, (Licensing Program Analyst) LPA Clayton conducted an unannounced visit to deliver the findings on the above allegation. LPA Clayton was greeted by Licensee Cheryl Muhammad. LPA Clayton toured the CCC for a Health and Safety inspection and observed 26 children being supervised and cared for by 6 fingerprint cleared staff.

On 09/04/2024 ESCCRO received a complaint alleging that Licensee enrolled a child without the necessary documentation.

On 09/10/2024 LPA Clayton conducted an unannounced 10 day inspection where LPA Clayton advised licensee of the complaint allegations, toured the facility for a Health and Safety inspection, interviewed staff and children, reviewed children files and obtained a copy of the Childcare facility Roster.

On 10/02/2024 LPA Clayton returned to the CCC and conducted additional file reviews.

LPA Clayton conducted a full investigation, which included facility visits, conducting interviews with the Reporting Party (complainant), interviews with staff, and file reviews.

During LPA Clayton's reviews of the children’s files, the documents mentioned in the complaint were observed in the child’s file, therefore the above allegation(s) is found to be UNSUBSTANTIATED, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur.



Exit interview conducted and report was reviewed with Licensee Cheryl Muhammad. LPA Clayton posted a Notice of Site visit which is to remain posted for 30 days.
