Licensing Program Analysts (LPAs), Joselito L. Del Mundo and Isabel Ortega met with Susan Gaines, lead teacher for the purpose of conducting an unannounced annual random inspection. Shortly after, School Readiness Coordinator Joe Vega-Smith and Assistant Administrator Lisa Fowler arrived at the facility. This program is held on the grounds of Wilsona Elementary School, in classroom A and B. This Head Start program operates Monday – Thursday, 7:35 am to 3:45 pm for classroom A and Monday, Tuesday, Thursday, Friday, 7:45 – 2:45 pm, and Wednesday from 7:45 – 1:45 pm for classroom B . Children in care are between the ages of 3 - 5 years of age. Present in classroom A were 15 children and 1 teacher, 1 aide and Classroom B had 17 children with 1 teacher, 1 aide. Teachers and staff are certified pediatric CPR/First Aid valid until 7/20/23.


The center consists of 2 large classrooms. Children's restrooms are located inside the classrooms. There are 2 toilets and 1 sink inside the bathrooms and 1 additional sink, directly outside each bathroom door. A separate staff restroom is located across from the classroom. Drinking water is available inside the classroom in the form of a water dispenser and disposable cups. There is a kitchen/storage room accessible from the office area of the classroom. There is a refrigerator inside the classrooms. Meals are prepared in the Elementary school cafeteria and are delivered 2 times a day by school personnel. LPAs observed plenty of perishable and non-perishable foods. The facility provides morning snack, breakfast, lunch and afternoon snack.

All utensils are cleaned by the cafeteria. Cleaning chemicals are stored under the sink in the staff's storage area located between the two classrooms. Children's medications are stored in locked boxes in classroom B. Isolation area for sick children is located between classroom A and B. First aid supplies are located in the classroom by the door exit. Storage for children's belongings & bathroom

facilities were inspected. Fire/Earthquake drills documentation were available and current and was conducted last February 24, 2003 at 11:45 am.

LPA observed fire extinguisher & evacuation plan posted. Center has an emergency storage room. Care & Supervision issues were addressed.



Trash cans have tight-fitting lids for solid waste disposal. Storage areas were inspected for toxins/cleaning compounds inaccessibility. Chemicals are stored in between the classrooms underneath the sink. Food and cleaning products were stored separately.

Outdoor play area was inspected and completely fenced in. The yard is accessible from the rear door of each classroom which gives direct access to the yard. The play area consists of play equipment appropriate for young children. There is a large play structure with rubber pore in place used for cushioning material, with 2 large slides, 1 small slide and various climbing steps. There is a swing set containing 6 swings, securely mounted over wood chips and rubber mats. There is a large grass area with trees that provide shade. There is also a large cement area with a bicycle path. There are 3 exit gates; one of which is locked and the other 2 have latches at the tops of the gates. A water pitcher and disposable cups will be taken outside during outdoor play time. There are no bodies of water in the play area.

All staff are fingerprinted through the school district. Staff files are maintained in the Human Resource of the Palmdale School District. Children's file are kept electronically. LPAs observed smoke detectors, carbon monoxide detectors and evacuation plans in each room. There is a Parent Board located at the entrance of the classroom.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. **Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). Family child care homes shall post during hours of operation. **Failure to meet the posting requirements shall result in an immediate $100.00 civil penalty. In addition; all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of report and sign the LIC 9224 acknowledging receipt. Civil Penalty assessments will be assessed if all above requirements are not adhered to. Applicant is aware of required forms for children's files and forms that shall be posted after being licensure.

**§1597.622 Employees or volunteers at family day care home; immunization requirements; records; exemptions. (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. – §1596.8662 Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion


(b) (1)   On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements


Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed child care facility are aware of situations that present the greatest danger to children.

Overall, the Center is in compliance per Title 22 regulations. During today's inspection, LPA Del Mundo and LPA Ortega did not observe any deficiencies that may hinder the health and safety of the children in care. No deficiencies were cited during inspection. An exit Interview, discussion of observations and this report were conducted.
