25 | Licensing Program Analyst (LPA) Roberto Luque-Avila and Licensing Program Manager Rita Ramos conducted an unannounced case management inspection on 05/24/24. LPA and LPM arrived at the facility and met with Lynn Moore, Teacher, who guided LPA and LPM on a tour of the facility. LPA and LPM were then met by Arina Faynshteyn. There were 10 children and 3 staff present upon arrival.
The purpose of the visit was to inspect the recently remodeled outdoor yard and play structure. The facility was previously granted a temporary outdoor waiver to use a parking area as an outdoor yard while the construction of the newly remodeled yard was occurring.
LPA and LPM inspected the outdoor yard, took measurements and pictures.
LPA and LPM did not observe any hazards and measurements taken still render space for the licensed capacity. Facility is cleared to use the newly remodeled outdoor yard and after today’s date the temporary waiver in place is no longer valid.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Arina Faynshteyn, Director, including, but not limited to Provider Rights, Appeal Procedures
