On 11/7/24 Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Inspection at the above facility. At 8:45AM LPA met with Porchia Henderson, Licensee, provided LIC 126 Entrance checklist, and was guided on tour of the home. LPA observed 3 children in care, including 2 infants. Co Licensee Michelle Vance was present in the home. Per Licensee there are 14 children currently enrolled. Per Licensee, hours of operation are Monday – Friday 6Am-6PM. Currently there are 2 children requiring Incidental Medical Services in care. At 9:00AM LPA observed medication in a red secured first aid kit labeled. Licensee provides breakfast, lunch, dinner and AM/PM snack. Per licensee the only outside food permitted is infant formula/ baby food supplied by parents. LPA reminded licensee that all outside food brought in by children should be dated and labeled with child’s name. Per Licensee she is participating in the food program. Per licensee, transportation service to and from school is provided. Per Licensee overnight care is not provided at this time. LPA observed all appropriate documents on licensing board (Facility License (LIC 203), Emergency Disaster Plan (LIC610a), last fire drill conducted 11/6/24, Notification of Parents' Rights Poster (PUB 394), Child Care Facility Roster (LIC9040). LPA observed Licensee Michelle and Porchia’s current Pediatric CPR (Adult/Infant /Child) and Pediatric First Aid certifications expiration date of 11/25 and licensee Michelle and Porchia’s Mandated Reporter Certificate expiration date of 10/26.
Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:
This is a single-story home consisting of five bedrooms, two bathrooms, living room, dining room, kitchen, den, detached Garage, and enclosed Backyard. Per Licensee areas used by children include: four bedrooms, one bathroom, and the backyard. Areas off limits children include one bedroom, kitchen, one bathroom, and garage. LPA did not observe baby gates blocking off kitchen. Per licensee all children in care are supervised 100% and know not to enter the kitchen. LPA advised Licensee to place baby gates in kitchen to make it inaccessible.

All poisons are kept in a locked cabinet above washer and dryer inaccessible to children in care. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible to children in care. LPA observed a fully charged 210ABC fire extinguisher LPA receipt on the wall of the daycare den, with no service tag or receipt. Per licensee she just purchased the fire extinguisher and will email LPA receipt. A technical violation will be issued. At 9:30AM LPA observed a functioning combination smoke/carbon monoxide detector located in the hallway next to the children’s restroom. The home was observed to have central air and heating for safety and comfort. No Wall heaters or fire places were observed. The home maintains working telephone service via cellphone. Per licensee there are firearms on the premises. At 9:40AM LPA observed an automatic rifle secured in a locked fire arm box. At 9:41AM LPA observed ammunition to be stored separate in a locked ammunition box.

LPA observed 1 bedroom used for infants to have 2 cribs, 2 Infant mattress were observed to be firm with tightly fitted sheets with no loose objects or bumpers hanging, or attached to the cribs. Per licensee all sheets and blankets are washed by Licensee weekly or as needed. LPA observed 2nd bedroom (toddler room) to have 2 additional cribs with appropriate mattresses, tightly fitted sheets and no loose objects or hanging materials above. LPA observed 3 children including 2 infants and licensee Michelle in the den. Per Licensee Porchia room is used for eating and entertainment. LPA observed back bedroom (Classroom) to have age appropriate toys, learning materials and be free of hazards to children in care.

Kitchen area was observed to be clean and have no sharp objects or chemical compounds accessible to children in care. LPA observed kitchen knives in a drawer secured by a safety latch and inaccessible to children in care. LPA observed bathroom for children’s use to be clean, fully stocked and have no chemicals or hazards in the sink cabinet.

Outdoor play area in the backyard is fully enclosed with no hazards present to children. LPA Observed a playhouse/play kitchen, several age appropriate bikes/ scooters as well as children’s tables and chairs. LPA observed no bodies of water on the premises.

Staff records were reviewed and observed to be complete. LPA reviewed 5 children’s files and observed 2 out of 5 were missing immunizations. Per Licensee Porchia she will obtain records and email LPA. Technical violation will be issued. LPA observed LIC 9227 (Sleep plan) in all infant files. LPA observed a sleep log on Brightwheel app for infants in care that does not meet regulations. LPA consulted with licensee Porchia and provided a sample infant sleep log. A technical violation will be issued.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee Porchia Henderson, confirmed there are no Registered Sex Offenders living in the home. LPA completed the RSO profile in FAS.
