The facility is a one-story house. There is a fireplace in the facility, and it is covered with a glass door. The home has central heating and air conditioning units.
LPA observed safe and age-appropriate toys, board games, puzzles, and books. LPA observed three sleeping cots. Per licensee, licensee has additional sleeping cots in the garage. Electrical outlets are covered with safety plugs.
Bathroom: Toilet, sink and faucet were clean and operable. LPA did not observe razors and/or sharp objects, mouthwash, medications, perfumes, air freshener, nail polish and nail remover that will pose a health and safety risk to children in care. LPA observed a potty seat cover in the bathroom cabinet. The cabinet has a broken child safety lock. Licensee was advised to fix the broken child safety lock.
Licensee stated that when a child shows signs of illness, the child is separated from other children, either in the living room, dining room, or family room of the facility. Per licensee, parents are called immediately.
Outdoor: The backyard has no bodies of water. LPA did not observe bodies of water in the backyard. The outdoor play area is made of concrete and grass, and it has a covered patio. LPA observed a small slide, two bicycles, and rideable toy cars.
At the time of inspection, Licensee is aware of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing on the day of the incident and/or 24 hours of incident by telephone, fax and/or in writing to the Department. Licensee is familiar with the Unusual Incident Report form, LIC624B. The report on unusual incident/injuries can also be mailed to unusualIncidentreport@dss.ca.gov
Review of records to be maintained: LPA reviewed with licensee the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. LPA reviewed four children’s files, licensee, and assistant’s files. Based on records review, children’s files are incomplete. Child #3 and #4 have missing LIC700, LIC627, LIC 282, LIC995A, and immunization records. Licensee was advised to send Child #3 and #4 records to LPA. Licensee and assistants have current mandated reporter training. Licensee has current pediatric First Aid/CPR/training Licensee and assistants have proof of immunizations against Pertussis and Measles. However, Assistant #2 has missing TB skin test on file. Per Assistant #2, A#2 send the TB skin test on file to the licensee. Assistant #2 was advised to provide and send a current TB clearance to LPA. Also, Assistants #1 and #2 are missing LIC9052. LPA provided the licensee with copies of LIC9052 and advised the licensee to have Assistants #1 and #2 fill out the form and send it to LPA. |
