Neglect/Lack of Supervision: Staff did not ensure sufficient supervision was provided resulting in a child sustaining an injury while in care
Reporting Requirements: Facility staff falsified incident reports
Physical Plant: Staff do not ensure classrooms are kept in clean sanitary conditions at all times.
Personal Rights: Staff allow children to be left in soiled clothing for extended periods of time
Personal Rights: Staff does not ensure children are spoken to in an appropriate manner.

On 08/25/2025 Licensing Program Analyst (LPA) Cristina Castellanos made an unannounced visit to the above-mentioned facility for the purpose of delivering complaint findings. Upon arrival LPA met with Lead Teacher/Assistant Director T. Carter and discussed the purpose of the visit. Shortly after Directors Paige Patton and Stephanie Patton-Holloman arrived at the facility.

LPA toured the facility and observed 18 children in care with 4 staff members providing care and supervision. All staff members present during today’s inspection have fingerprint clearance.

On 06/18/2025 Licensing Program Analyst (LPA) Cristina Castellanos and Judy Laureano arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations.

During the course of the investigation LPAs Castellanos and Laureano requested and reviewed the following documents: children’s files, children's roster, personnel records, staff roster and accident reports for the months of April & May 2025.

On 08/20/2025 LPA Castellanos concluded interviews of all the relevant parties.

Based on LPA interviews of all relevant parties and record review, no information revealed that staff did not ensure sufficient supervision was provided resulting in a child sustaining an injury while in care, that facility staff falsified incident reports, that staff do not ensure classrooms are kept in clean sanitary conditions at all times, that staff allow children to be left in soiled clothing for extended periods of time and that staff does not ensure children are spoken to in an appropriate manner. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted with the Director. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
