On 03/20/2025 Licensing Program Analyst LPA Ranita Richmond and Chartrice Johnson arrived at the home to conduct a Plan of Correction visit and was met by Licensee Liliana. LPA observed 7 children in care being supervised and cared for by licensee and 1 fingerprint cleared assistant.

On 02/04/2025, Licensee was cited for the following:
1. Licensee will ensure that open face heaters shall be screened to prevent access by children.
2. Licensee will ensure that all detergents, cleaning compounds, etc. are inaccessible to children in care.
3. The provider shall check and document the all infants sleep, using an infant sleep log for all infants up to 24 months of age.
4. Licensee will ensure that personnel files contain a signed and dated copy of the Notice of Employee Rights LIC 9052.
5. Licensee will ensure that children's file contain a signed affidavit stating that the parent has been informed that the family child care home does not carry liability insurance, LIC 282.
6. Licensee will ensure that all staff have current immunization on file, including TB clearance.
7. Licensee will ensure that all cribs are free from all loose articles and objects.
8. Licensee will ensure all cribs are free from hanging on or attached items to the sides of cribs at all times.
9. Licensee will ensure a signed LIC 9227, for sections C and D are completed if an infant changes positions they are able to remain in the changed position.

During visit on 3/20/25 LPA Richmond and Johnson observed the following:
1. LPA observed that open face heater not to be screened to prevent access by children. Heater covered by artificial grass. Not cleared during today's visit.
2. LPA observed bleach in laundry area, cleaning compounds, disinfectant wipes and sprays, beauty supplies, etc. on shelves, inside medicine cabinet, and inside tub/shower areas in the bathroom that are accessible to children in care. Not cleared during today's visit.
3. LPA observed infant sleep charts for four infants completed for week of 3/17/25. Cleared during today's visit.
4.LPA observed personnel files contain a signed and dated copy of the Notice of Employee Rights LIC 9052. Cleared during today's visit.
5. LPA observed children's file contain a signed affidavit stating that the parent has been informed that the family child care home does not carry liability insurance, LIC 282. Cleared during today's visit.
6. LPA observed staff have current immunization on file, including TB clearance. Cleared during today's visit.
7. LPA observed that all cribs are not free from all loose articles and objects. LPA observed blankets, toys, clothes, and shoes inside of the cribs. Not cleared during today's visit.
8. LPA observed all cribs are not free from hanging on or attached items to the sides of cribs. LPA observed blankets hanging from the side of cribs. Not cleared during today's visit.
9. LPA observed signed LIC 9227, for sections C and D are completed if an infant changes positions they are able to remain in the changed position. Cleared during today's visit.

An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Licensee.
