25 | On 2/16/2024, at 03:00 p.m., Licensing Program Analyst (LPA)Isabel Ortega conducted an unannounced Case Management Incident inspection and met with facility Director Angie Serrano. LPA disclosed the purpose of the inspection and was granted entry into the facility. At the time of the inspection 60 children were present and nine staff providing care and supervision.
The purpose of the inspection was to follow up on a self reported unusual incident report (UIR) which occurred on January 24, 2024 at the facility. The Department was notified of the UIR on 1/24/2024. According to the UIR, there was an odor coming from the lounge room next to the teacher's break room. The odor was enclosed in the staff room by the double doors and the ceiling open vent was kept open. Poison control was contacted twice and went over urgent signs and symptom to evaluate staff and children. Staff #4 completed a walk through of the facility and reported back to the Director. Staff #3 evaluated staff symptoms according to the instructions provided by the poison control. Some staff only reported headaches and no children were affected. Per staff #3's interview, the Fire Department/paramedics cleared the facility about 2:00 p.m. No toxic chemicals were present at the facility and facility was deemed safe to operate.
LPA completed a safety inspection of the facility including three classrooms and the staff lounge. During today's inspection, LPA interviewed staff members, completed a walk through the facility. No further investigation is required. No deficiencies were observed.
An exit interview was conducted, a copy of this report, and notice of site visit were provided to the facility Director.
