On 3/1/2023 Licensing Program Analyst (LPA), Suzette Ornelas conducted an unannounced Annual Required Inspection for the cooperative facility. LPA met with Director, THRAILKILL ALVARADO, TAMMY and parent volunteer Kelly Barrios and toured the facility indoors and outdoors. Days and hours of operation are Monday through Thursday 9a to 12p.

LPA observed all required postings in a prominent area. A review of the sign in/out sheet was conducted to verify the current census of children. LPA observed 15 children and 7 adults. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classroom area and indoor play sheds was conducted. All areas identified on the Facility Sketch were inspected. Per director, the program is outdoor based; however, LPA observed 1 licensed preschool classroom on the premises.

A walk through of the classroom room space was conducted. LPA reminded director to ensure container bins are not piled up on top shelves to prevent falling. The space was found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. LPA observed wallpaper to be peeling off wall located towards the back of the classroom, some floor tile in need of repair and discolored ceiling tile in need of repair. Per director, company that owns the modular will be contacted to assist with repairs during spring break (4/3/2023) when children are not present. Director will provide pictures of repairs made to LPA via email on or before 4/13/2023. There is adequate heating, lighting and ventilation. Isolation area for sick students is located in the indoor classroom near the restrooms.

The bathroom and toileting areas were inspected, LPA observed toilets and sinks to accommodate the facility’s capacity. Facility is equipped with 2 toilets and 5 sinks, 3 are located inside the classroom and 2 are located outdoors. Toilets flush properly, toilet and sinks are reachable by the children in care. The restrooms have adequate toilet paper and paper towels available. Hot water is turned off in the children's restrooms.

The restrooms were found to be clean. There is adequate lighting/ventilation in the bathroom area.

Playground equipment is in safe condition and free of hazards. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. Per director, parents provide daily meals/snacks for children. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is readily available both indoors and outdoors, each child has their own refillable water container. Per director, Water company (Sparkletts) provides monthly service.

The facility is equipped with a working smoke detector and carbon monoxide detector in every building. LPA observed a fire extinguisher (2A 10BC) located in the classroom with a service date of 02/2022. LPA informed director that fire extinguisher must be serviced yearly serviced. Per director, fire extinguisher will be serviced on or before 3/20/2023. There is no swimming pool or other bodies of water on the premises. First Aid Kit was observed to be complete. Per Assistant, emergency drills are conducted twice a month. LPA observed emergency drill schedule for 2023 located in the classroom. LPA provided director/assistant with a sample drill log to ensure drills are documented moving forward. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible by being kept in the staff restroom in a latched cabinet. No poisons were observed during the inspection.

LPA reviewed a sample of children’s files and observed files were complete. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff files and observed files were incomplete. On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training (www.mandatedreporterca.com). Director will provide proof of completion to LPA via email on or before 3/20/2023.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
