On 8/7/2024 at 10: 47a.m. LPA Whitmore initiated the complaint investigation and met with the Teacher Fabiola Ruiz. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with a total of 15 children and 3 teachers. LPA obtained a copy of Personnel Report, Facility Roster, Allergy list & Case Notes. LPA Whitmore interviewed staff and children and reviewed one file.The Department conducted a full investigation, which included staff interviews, interviews with relevant parties as well as a record review, including documentation related to the allegation. With the information obtained, LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegation of Personal Rights - Staff did not accord child’s dignity in their relationship with staff or other and Staff inappropriately punished day care child. Therefore, the allegations are unsubstantiated meaning although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. An exit interview was conducted, a copy of this report, appeal rights along with Notice of Site Visit were provided.
