25 | On 09/26/2023 at 1:30 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced Case Management inspection to follow up on an incident that was reported to The Department on 09/15/2023. Upon arrival, LPA met with Licensee, Brittie Crawford, and Debora Cornejo, Director, who provided LPA a tour of the facility inside and outside. Census was taken.
On 09/15/2023, an incident was reported to The Department where a child was running and tripped landing on his arm, child complained of pain in his arm. Parents were called and child went to hospital for a check up where they reported child suffered a contusion (bone bruise). Per Director, child only attends twice a week and did not attend for a week following the incident but has since returned to program. LPA observed child at time of today's inspection and observed child playing at the facility. LPA observed the indoor classroom where the incident took place and did not observe any hazards at time of inspection.
The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days. Based on the information obtained and LPA observation of the play area, LPA determined there was adequate care and supervision provided at the day of incident.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited in regards to incident.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Director, Debora Cornejo.
