On 4/8/26 at 8:08am Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced initial complaint investigation regarding the above allegation. Upon arrival, LPA was guided on a tour of the facility by Director/Licensee Anna Shirokova, to whom the purpose of the inspection was announced. LPA Janet Gill arrived at 10am.

LPA toured the facility and observed 23 children outside on the playground being supervised by 2 staff.
During the investigation, LPA Lipsey made observations of the classrooms and kitchen, interviewed two staff and took photos and video.

Pertaining to the allegation, “Staff are not appropriately addressing pest infestation in the facility”
Per reporting party, they observed cockroaches in the kitchen where food is being prepared.

This is an amendment of an original report created on 4/8//26.  The report has been amended to change remove personal and correct dates.

LPA toured and inspected the classrooms and the kitchen. LPA observed and took photos of three dead cockroaches located behind the washer, under the refrigerator and in a bottom cabinet that housed blue latex gloves. LPA observed and took video of a 2 live cockroaches crawling on a shelf in the kitchen where food is being prepared.

The Reporting Party reported the pest issue to the staff #1 S1 on 3/25/26. Per S1, a company came out to spray on the weekend of 3/29/26 while the children were not present. Staff did not provide invoices or receipts of the spraying. Per staff #2, they have a cleaning company that comes on Wednesday and Sunday or as needed. S2 will have the cleaning crew do a more thorough cleaning of the kitchen. Per S1 pest control will be scheduled monthly until the pest are under control.

Based upon evidence obtained during this investigation, the above allegation, “Staff are not appropriately addressing pest infestation in the facility” has been determined to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standards has been met.

A Type A citation is being issued. See LIC9099-D for deficiency cited. LPA Jeanine Lipsey informed Director/Licensee Anna Shirokova that this report dated 4/8/26 documents One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, Safety, or personal rights of children in care.



Also, LPA Jeanine Lipsey informed the licensee to provide a copy of this licensing report dated 4/8/26 that documents any Type A citation to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9924), or written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director/Licensee Anna Shirokova. Notice of site visit was given and must remain posted for 30 days.

 Per reporting party, Staff sprayed toxic chemicals directly on or near food preparation surfaces.

Per both staff, they never saw a live cockroach, so they did not use any chemicals to kill the pest. They would use a bottle of disinfectant to clean the counter tops. S1 stated the kitchen personal was allergic to chemicals and they did not use any other products to kill them. LPA observed and took photos of the disinfectant, “Citrus II” they used to clean the counter. LPA did not observe any other hazardous chemicals on or near the counter tops.

Based upon evidence obtained during this investigation, the allegations “Staff utilized chemicals in an inappropriate manner resulting in possible contamination of food preparation surfaces have been determined to be unsubstantiated. A finding that a complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred.

Exit interview conducted and report was reviewed with Director/Licensee Anna Shirokova. Notice of site visit was given and must remain posted for 30 days.

Amended report dated 4/8/26 to correct the deficiency.

The Director will supply proof of the company coming out monthly to control the pest. Director will have the pest control come this month April and in May and send proof by POC date.

Based on observation and interviews, the licensee did not comply with the section sited above in that Licensee did not ensure the center was clear of cockroaches, which poses a immediate health, safety or personal risk to children in care.
