LPA toured and inspected the classrooms and the kitchen. LPA observed and took photos of three dead cockroaches located behind the washer, under the refrigerator and in a bottom cabinet that housed blue latex gloves. LPA observed and took video of a 2 live cockroaches crawling on a shelf in the kitchen where food is being prepared.
The Reporting Party reported the pest issue to the staff #1 S1 on 3/25/26. Per S1, a company came out to spray on the weekend of 3/29/26 while the children were not present. Staff did not provide invoices or receipts of the spraying. Per staff #2, they have a cleaning company that comes on Wednesday and Sunday or as needed. S2 will have the cleaning crew do a more thorough cleaning of the kitchen. Per S1 pest control will be scheduled monthly until the pest are under control.
Based upon evidence obtained during this investigation, the above allegation, “Staff are not appropriately addressing pest infestation in the facility” has been determined to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standards has been met.
A Type A citation is being issued. See LIC9099-D for deficiency cited. LPA Jeanine Lipsey informed Director/Licensee Anna Shirokova that this report dated 4/8/26 documents One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, Safety, or personal rights of children in care.
Also, LPA Jeanine Lipsey informed the licensee to provide a copy of this licensing report dated 4/8/26 that documents any Type A citation to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9924), or written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with Director/Licensee Anna Shirokova. Notice of site visit was given and must remain posted for 30 days. |
