On November 21, 2023, Licensing Program Analysts (LPAs) Monique Ayala and Suzana Safaryan conducted an unannounced required 3-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Director, Zahava Block who guided the LPAs on a tour of the facility. This is a preschool age program that operates Monday to Friday 8:30am to 4:15pm. Per director there are 158 children enrolled. LPAs observed 136 children.

LPAs observed a security that is armed patrolling the outside perimeter of the facility and then entered the facility to place his jacket away. The armed security is placed at the preschool due to the increase threat and violence in the Jewish community. Per the director, the armed security is present when children are being dropped off during car-pool. LPAs spoke with security who stated he was been placed at this facility for about a month and is there Monday to Friday from 8am to 4pm. The security stated he works for Global Security. Per director, parents were notified of the armed security via, email. Director provided LPAs with a copy of the email. The facility is in the process of requesting a waiver for the armed security. LPAs observed facility's parent handbook and did not observe anything stating there is an armed security at the facility.

This is a 3 story building with the third floor only having a play yard for children. The classrooms are located on the first and second floor. The following classrooms were inspected: Classroom 101: 14 children with 3 teachers; Classroom 102: 17 children with 2 staff; Classroom 103: 17 children with 3 teachers and Classroom 104: 10 children with 3 staff; Classroom 201: 16 children with 3 teacher; Classroom 202: 16 children with 4 teachers; Classroom 203: 17 children with 4 teachers;

Classroom 204: 15 children with 3 teachers; Kindergarten Classroom: 14 children with 3 teacher. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. Licensed facility is within the conditions, limitations, and capacity specified on the license.

At approximately 8:25AM, LPAs observed staff member sitting in a chair where her visual supervision was obstructed by a wall and a child was alone without supervision on the other side of the room. LPAs also observed the same staff member reading instead of providing supervision to children in care. The children were observed to be sitting as they waited to be disbursed to their classrooms.


The classroom furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. LPAs did not observe tight fitted lids on the trash cans. LPAs observed the napping equipment to be touching with other children's napping equipment.

The facility does not provide any meals to children in care as children bring their meals from home. Children meals are placed in their cubbies. The facility does provide water.

The facility did not get their water tested for lead as their building was built after 2010. LPA observed that there are functioning smoke and carbon detectors in the facility. LPA observed a fully charged and serviced fire extinguisher. Emergency disaster drill was last conducted on 11/13/23.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

SB792 (Immunization Requirements for Staff and Employees) was discussed with the Director. The Director and staff do currently have immunization documentation.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers. LPAs observed 6 staff members have an expired certificate.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

The facility is being cited a Type A deficiency. Upon Receipt, licensee shall post and provide copies of this licensing report and LIC9224 to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.



A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and a copy of the report was provided to director along with appeal rights.
