On 11/4/2021, Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced complaint visit for the purpose of concluding the investigation regarding the allegations above. LPA met with Licensee, Margaret Pettis, and explained the purpose of the inspection. LPA observed 19 children with 1 staff.

During this inspection, LPA interviewed 4 children (see LIC 811 Confidential Names) and obtained the sign in/out sheet for 11/4/2021.

On 8/27/2021, LPAs Lillian Casillas and Judy Laureano initiated the complaint investigation. LPAs met with former director, Michelle Nance, who led LPAs on a tour of the facility. LPAs obtained copies of the following documents: children’s roster (LIC9040) and sign-in/sign-out sheets for 8/27/2021.

On 9/10/2021, LPA Casillas conducted an unannounced follow-up complaint investigation regarding the allegation above. LPA obtained copies of the following documents: sign-in/sign-out sheets for 9/10/2021 and 8/30-9/9/2021, staff contact information and schedule, and the LIC 308 Designation of Facility Responsibility LPA met with Licensee, Margaret Pettis, and interviewed 3 children and 3 staff.

Based on the investigation, which included interviews with relevant parties, observation, and record review, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted and a copy of this report along with the Notice of Site Visit and Appeal Rights were provided to Licensee, Margaret Pettis.
