On September 27, 2024, at 12:45 PM Licensing Program Analysts (LPAs) Priscilla Ochoa and Nolan Tchang conducted an unannounced annual inspection at the above facility. A COVID-19 risk assessment was conducted prior to entering the facility. Upon arrival LPAs rang the callbox doorbell and were granted entry to the front patio and walked to the side gate which leads to the backyard. LPAs were greeted by licensee, Karina Torres. Licensee’s assistant was also present for the inspection. Licensee has 14 children enrolled and LPAs observed 7 children in care. Hours of operation are Monday through Friday, 8:00 AM to 6:00 PM.

Family members residing in the home are 2 adults who have criminal record clearance and licensee’s minor child. This is a single-story house that consist of 3 bedrooms, 3 bathrooms, living room, dining room, kitchen, pantry, laundry room located in kitchen, back yard which is fenced and detached garage.

Areas accessible to children are 1 bedroom, 1 bathroom, backyard and detached garage. The bedroom is located towards the back of the house with a door leading to the back yard. Per licensee, this bedroom is used for napping. The bathroom is located in the napping room. The detached garage as a classroom area and not used to napping or meal time. Licensee is part of a meal program through child resources and provides all meals and snacks for children in care.

Off limit areas include 2 bedrooms, 2 bathrooms, living room, dining room, kitchen, pantry, laundry room. Per licensee there are 2 cats that are located in the home away from day care children.

Licensee guided LPAs on a tour of the areas that are accessible to the children in care. Once entered the side gate; the detached garage and back patio were observed. Detached garage is used as a classroom where children partake in activities. In the patio area and classroom there are several age-appropriate toys observed. Parents sign in and out their child by scanning a QR code that is located on the glass door of the classroom and documented on PlayGround App. LPAs were then guided up a ramp leading to the napping room. In the napping room, LPAs observed napping equipment in the form of mats. Per licensee, children take home their sheets for washing every Friday. LPAs advised licensee to rearrange napping equipment as mats were blocking back door access. Licensee and Assistant made sure, mats were arranged in an orderly manner preventing mats from blocking back door entry / exist while children are napping. In the napping room LPAs went through a walkway to access the bathroom. LPAs did observe clearing solutions in a cabinet above the bathroom sink, which were in assessable to children in care. Next, LPAs observed the back yard which consist of a sand box next to the ramp leading to the napping room. Per licensee, the sandbox is covered with a tarp every night to maintain it clean. A play structure with slides and swings age appropriate for children in care was also observed. All areas were observed to be clean, orderly, comfortable, and well ventilated.

Per licensee, the napping room will be used as an isolation area when a child falls ill. A mat will be provided if needed for child in care.

Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children. LPA observed working smoke detector and Carbon Monoxide. Fire extinguisher was observed in the classroom to be expired as of 8/25/2023. LPAs observed licensee purchased a new 2A-10BC fire extinguisher during todays visit. There is a working telephone on site. Daycare parents have access to the landline and licensee’s cellphone number during daycare hours.

Per the licensee, there are no firearms on the premises. Licensee has current First Aid and CPR, expires 12/2025.

LPAs reviewed children files; files were found to be complete. LPA reviewed staff files. Staff #1 has an expired mandated reporter training. Other observed staff files were complete. Children and staff names were documented on LIC811 (confidential page).

All childcare employees must complete mandated reporter training at www.mandatedreporterca.com, within 30 days of hire and renew training every two years.

Per the licensee, fire and disaster drills are conducted every 6 months; last drill documented and conducted on 9/10/2024.

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Emergency Disaster Plan (LIC610a), Notification of Parents' Rights Poster (PUB 394).

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B. The on Duty Worker is available for questions Monday through Friday at (323) 981-3350 from 8:00 AM - 5:00 PM.

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

The licensee was informed that the presence of adults in the home without Criminal Record Clearance or Exemption will be cited and civil penalty assessed for $100 per day. The licensee may find additional information and forms on the DSS website at www.ccld.ca.gov including information on the Live Scan application (LIC 9163). Appointments can be made for Live Scan at 1-800-315-4507

The following was discussed with the licensee:


Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. Licensee was reminded that supervision is always required to children in care.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The licensee, Karina Torres confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

LPA provided consultation during inspection.



At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies being issued today, September 27, 2024.

A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and a copy of the report was provided, along with appeal rights to licensee, Karina Torres.
